羟基喜树碱联合奈达铂对鼻咽癌患者临床效果及Th1、Th2型细胞因子水平的影响

Influence of hydroxycamptothecin combined with nidaplatin on the clinical effect and levels of Th1 and Th2 cytokines in patients with nasopharyngeal carcinoma

目的探讨羟基喜树碱联合奈达铂对鼻咽癌患者临床效果及Th1、Th2型细胞因子水平的影响。方法将2015年2月至2018年11月收治的120例鼻咽癌患者随机分为联合组与奈达铂组,各60例。奈达铂组采取奈达铂联合同期适形放疗,联合组在奈达铂组基础上联合羟基喜树碱治疗。比较两组的治疗效果、Th1、Th2型细胞因子水平及不良反应发生情况。结果联合组治疗总有效率显著高于奈达铂组(P<0.05)。治疗后,联合组的IL-2、IL-5、IFN-γ水平高于奈达铂组,TNF-α、IL-4、IL-10水平低于奈达铂组(P<0.05)。两组患者的各不良反应发生率比较,差异无统计学意义(P>0.05)。结论羟基喜树碱联合奈达铂配合同期适形放疗治疗鼻咽癌可提高总有效率,且不会明显增加不良反应,值得推广。

Objective To investigate the influence of hydroxycamptothecin combined with nedaplatin on clinical effect and the levels of Th1 and Th2 cytokines in patients with nasopharyngeal carcinoma. Methods A total of 120 patients with nasopharyngeal carcinoma who were admitted from February 2015 to November 2018 were randomly divided into combination group and nedaplatin group, with 60 cases in each group. The nedaplatin group was treated with nedaplatin combined with concurrent conformal therapy, and the combination group was treated with hydroxycamptothecin on the basis of the nedaplatin group. The therapeutic effects, levels of Th1 and Th2 cytokines and adverse reactions were compared between the two groups. Results The total effective rate of treatment in the combined group was significantly higher than that in the nedaplatin group (P<0.05). After treatment, the levels of IL-2, IL-5 and IFN-γ in the combined group were higher than those in the nedaplatin group, while the levels of TNF-α, IL-4 and IL-10 were lower than those in the nedaplatin group (P<0.05). There were no significant differences in the incidences of various adverse reactions between the two groups (P>0.05). Conclusion Hydroxycamptothecin combined with nedaplatin and conformal therapy for nasopharyngeal carcinoma can improve the total effective rate without significantly increasing adverse reactions, which is worthy of promotion.
