早期胃癌的胃镜活检病理检查结果及临床病理特点

Pathological examination results and clinicopathological characteristics of gastroscopic biopsy for early gastric cancer

目的分析早期胃癌的胃镜活检病理检查结果及临床病理特点。方法选取我院2016年1月至2019年1月病理检查确诊为早期胃癌的60例患者为研究对象,分析早期胃癌临床病理学特点以及胃镜活检与术后病理学诊断结果的一致性,并检测血管内皮生长因子(VEGF)。结果胃镜活检诊断早期胃癌患者57例,与术后病理学诊断基本一致。病理学检查结果显示,75.00%的患者病灶直径<3 cm;63.33%的患者浸润程度为黏膜内;51.67%患者为中分化腺癌;淋巴结转移8例;胃组织病理检查VEGF阳性7例,且均发生在淋巴结转移组。淋巴结转移组VEGF阳性率显著高于非淋巴结转移组(P<0.05)。结论胃镜活检结果与病理检查具有较高的一致性,同时检测VEGF有助于判断淋巴结转移情况。

Objective To analyze the pathological examination results and clinicopathological characteristics of gastroscopic biopsy for early gastric cancer. Methods A total of 60 patients with early gastric cancer diagnosed by pathological examination admitted in our hospital from January 2016 to January 2019 were selected as the research objects. The clinical pathological features of early gastric cancer and the consistency of endoscopic biopsy and postoperative pathological diagnosis were analyzed, and vascular endothelial growth factor (VEGF) were detected. Results There were 57 patients with early gastric cancer diagnosed by gastroscopic biopsy, which was basically consistent with the pathological diagnosis after operation. Pathological examination results showed that there were 75.00% of patients with the lesions diameter<3 cm, 63.33%of patients with the intramucosal infiltration degree, 51.67%of patients with moderately differentiated adenocarcinoma and 8 cases with lymph node metastasis. Gastric histopathology showed that 7 cases were positive for VEGF and all occurred in lymph node metastasis group. The positive rate of VEGF in lymph node metastasis group was significantly higher than that in non-lymph node metastasis group (P<0.05). Conclusion Gastroscopic biopsy results have a high consistency with pathological examination, and detection of VEGF is helpful to determine lymph node metastasis.
