腹腔镜与胆道镜联合取石术治疗肝内胆管结石的临床效果

Clinical Effect of Laparoscopic Choledochoscopy Combined with Lithotomy in the Treatment of Intrahepatic Bile Duct Stones

目的:探究肝内胆管结石行腹腔镜联合胆道镜取石术治疗的临床效果及安全性。方法:择取2018年1月~2019年1月期间某院收治的50例肝内胆管结石患者参与研究,通过抛硬币方式进行随机分组,其中对照组和观察组各25例。对照组执行开腹手术+胆道镜手术治疗,观察组执行腹腔镜+胆道镜手术治疗,比较两组患者围手术期指标、结石清除率和并发症情况。结果:观察组患者手术时间、出血量、排气时间和住院时间相比对照组更少,术后并发症发生率8.00%,显著低于对照组32.00%,数据差异有统计学意义(P<0.05);观察组和对照组结石清除率分别为100.00%和96.00%,数据差异无统计学意义(P>0.05)。结论:腹腔镜联合胆道镜取石术对肝内胆管结石患者具有良好的临床效果和治疗安全性,因此该种治疗方法可用于临床普及。

Objective:To investigate the clinical efficacy and safety of laparoscopic choledochoscopy combined with lithotomy in the treatment of intrahepatic bile duct stones. Methods:50 patients with intrahepatic bile duct stones admitted to a hospital from January 2018 to January 2019 were enrolled in the study. They were randomly divided into two groups by coin-throwing method,including 25 cases in the control group and 25 cases in the observation group. The control group underwent laparotomy + choledochoscopy,and the observation group underwent laparoscopic + choledochoscopy. The perioperative indicators,stone clearance rates,and complications were compared between the two groups. Results:The operation time,blood loss,exhaust time and hospitalization time of the observation group were less than those of the control group. The postoperative complication rate was 8.00%,which was significantly lower than that of the control group(32.00%),the difference was statistically significant(P<0.05). The stone removal rates of the observation group and the control group were 100.00% and 96.00%,respectively,and the difference was not statistically significant(P>0.05). Conclusion:Laparoscopic combined with choledochoscopy has good clinical effect and safety in patients with intrahepatic bile duct stones. Therefore,this kind of treatment can be used for clinical popularization.
