我院静脉用药调配中心不合理处方分析

Analysis of irrational prescription status of the intravenous drug dispensing center in our hospital

目的:了解山东大学齐鲁医院静脉用药调配中心不合理处方的类型、分布等情况。方法:回顾性分析2017年5月至2018年12月期间1596例静脉用药不合理处方的类型、分布等情况。结果:各科室存在的不合理用药情况为,神经内科258份(16.17%)、心内科189份(11.84%)、骨科172份(10.70%)、神经外科164份(10.28%)、妇产科136份(8.52%)、消化内科128份(8.02%)、肾脏内科124份(7.77%)、ICU119份(7.46%)、血液内科117份(7.33%)、儿科83份(5.20%)、普外科64份(4.01%)、心胸外科42份(2.63%),不合理用药情况分布比较多的科室分别是神经内科、心内科、骨科和神经外科。不合理处方的类型为,给药浓度不合理、配伍禁忌、输液溶媒选择不当、给药频次不合理以及其他方面,所占比例依次为53.09%(833/1569)、19.99%(319/1596)、17.29%(276/1596)、4.95%(79/1596)、5.58%(89/1596)。结论:静脉用药不合理处方和所及科室均较普遍,不合理处方主要类型为给药浓度、配伍禁忌和输液溶媒。

Objective:To understand type and distribution of irrational prescriptions in our hospital’s intravenous drug dispensing center. Methods:1596 cases of irrational prescriptions in our hospital from May 2017 to December 2018 were retrospectively analyzed to explore the type and distribution of these prescriptions.Results:The irrational prescriptions were 258 cases (16.17%) in neurology department, 189 (11.84%) in cardiology department, 172 (10.70%) in orthopedics department, 164 (10.28%) in neurosurgery department, 136 (8.52%) in obstetrics and gynecology department, 128 (8.02%) in digestive system department, 124 (7.77%) in nephrology department, 119 (7.46%) in ICU, 117 (7.33%) in hematology department, 83 (5.20%) in pediatrics department, 64 (4.01%) in general surgery department, and 42 (2.63%) in cardiothoracic surgery department. The departments with more irrational drug distribution were neurology, cardiology, orthopedics and neurosurgery.The types of irrational prescriptions were unreasonable drug concentration, incompatibility, inappropriate selection of infusion solvent, unreasonable frequency of administration, and other aspects, accounting for 54.70%(873/1569), 19.99%(319/1596), 17.29%(276/1596), 4.95%(79/1596), 3.07%(49/1596), separately. Conclusions:The irrational prescriptions are common in our hospital and are involved in almost all departments.The main types of irrational prescriptions are drug concentration, incompatibility and infusion solvent.
