半夏泻心汤联合三联疗法治疗消化性溃疡(肝脾不和)疗效观察

Observation of Curative Effect of Banxia Xiexin Decoction combined with Triple Therapy in Treatment of Peptic Ulcer (Disharmony between Spleen and Liver)

[目的]观察半夏泻心汤联合三联疗法治疗消化性溃疡(肝脾不和)疗效。[方法]将121例门诊患者按就诊顺序号随机分为两组;禁辛辣刺激饮食、禁酒。对照组60例阿莫西林,0.5 g/次,3~4次/d,最大剂量<4 g/d;左氧氟沙星,0.2 g/次,2次/d,连用7~14 d;雷贝拉唑,10 mg/次,1~2次/d。治疗组61例半夏泻心汤,1剂/d,早晚温服;西药治疗同对照组。连续治疗30 d为1疗程。观测临床表现、溃疡面积、中医证候积分、不良反应。治疗1疗程(30 d),判定疗效。[结果]治疗组显效33例,有效26例,无效2例,总有效率96.73%;对照组显效28例,有效21例,无效11例,总有效率81.67%;治疗组疗效优于对照组(P<0.05)。中医证候积分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。不良反应率治疗组低于对照组(P<0.01)。[结论]半夏泻心汤联合三联疗法治疗消化性溃疡(肝脾不和),疗效满意,不良反应更低,值得推广。

[Objective]To observe the curative effect of Banxia Xiexin Decoction combined with triple therapy on peptic ulcer(disharmony between spleen and liver).[Method]Totally 121 outpatients were randomly divided into two groups according to the order number of visits:no pungent stimulating diet and no alcohol.The control group’s 60 patients were treated with amoxicillin with the dose of 0.5 g/time,3~4 times/day(the maximum dose was less than 4 g/day),levofloxacin with the dose of 0.2 g/time,2 times/day for 7~14 days,rabeprazole with the dose of 10 mg/time,1~2 times/day.A total of 61 patients in the treatment group were treated with Banxia Xiexin decoction,1 dose/d,taking in the morning and evening.The western medicine treatment was the same as that of the control group.Continuous treatment for 30 days was a course of treatment.Clinical manifestations,ulcer area,TCM syndrome score and adverse reactions were observed.One course of treatment(30 days)was used to determine the curative effect.[Result]Totally 33 cases were markedly effective in the treatment group,26 cases were effective,2 cases were ineffective,and the total effective rate was 96.73%.Totally 28 cases were markedly effective in the control group,21 cases were effective,11 cases were ineffective,and the total effective rate was 81.67%.The curative effect of the treatment group was better than that of the control group(P<0.05).The scores of TCM syndromes were improved in both groups(P<0.01),and the improvement in the treatment group was better than that in the control group(P<0.01).The adverse reaction rate in the treatment group was lower than that in the control group(P<0.01).[Conclusion]Banxia Xiexin decoction combined with triple therapy in the treatment of peptic ulcer(disharmony between spleen and liver)has satisfactory curative effect and lower adverse reactions,which is worthy of promotion.
