清气化痰汤雾化吸入联合西药治疗急诊呼吸机相关性肺炎疗效观察

Observation on the Therapeutic Effect of Aerosol Inhalation of Qingqi Tantang Decoction combined with Western Medicine in the Treatment of Emergency Ventilator-associated Pneumonia

[目的]观察清气化痰汤雾化吸入联合西药治疗急诊呼吸机相关性肺炎疗效。[方法]将94例患者按就诊顺序号法分为对照组和治疗组各47例。对照组亚胺培南西司他丁钠0.9 m L+生理盐水100 m L,2次/d,静滴;氨溴索30 mg/次,2次/d,雾化吸入。治疗组加用清气化痰汤,1剂/d,水煎100 m L,雾化吸入,吸入速率5 m L/min,2次/d。连续治疗10 d为1疗程。观测临床表现、体温、白细胞计数、PCT、APACHE评分、不良反应。[结果]治疗组痊愈13例,显效17例,有效15例,无效2例,总有效率95.74%;对照组痊愈10例,显效15例,有效13例,无效9例,总有效率80.85%;治疗组疗效优于对照组(P<0.05)。体温、白细胞计数、PCT、PACHE评分两组均有改善(P<0.05),治疗组改善优于对照组(P<0.05)。[结论]清气化痰汤雾化吸入联合西药治疗急诊呼吸机相关性肺炎,可提高临床疗效,无严重不良反应,值得推广。

[Objective]To observe the efficacy of Qingqi Huatan decoction aerosol inhalation combined with western medicine in the treatment of emergency ventilator-associated pneumonia.[Method]94 patients were divided into control group and treatment group according to the order of visiting.In the control group,imipenem cilastatin sodium 0.9 m L+saline 100 m L,twice a day,intravenous drip;ambroxol 30 mg/time,twice a day,aerosol inhalation.The treatment group was treated with Qingqi Huatan decoction,1 dose/d,decoction 100 m L,aerosol inhalation,inhalation rate 5 m L/min,2 times/d.Continuous treatment for 10 days is a course of treatment.Clinical manifestations,body temperature,white blood cell count,PCT,APACHE score and adverse reactions were observed.[Result]In the treatment group,13 cases were cured,17 cases were markedly effective,15 cases were effective and 2 cases were ineffective,the total effective rate was 95.74%.In the control group,10 cases were cured,15 cases were markedly effective,13 cases were effective and 9 cases were ineffective,the total effective rate was 80.85%.The curative effect of the treatment group was better than that of the control group(P<0.05).Temperature,white blood cell count,PCT and PACHE scores were improved in both groups(P<0.05),and the improvement in treatment group was better than that in control group(P<0.05).[Conclusion]Qingqi Tan Decoction aerosol inhalation combined with western medicine in the treatment of emergency ventilator-associated pneumonia can improve the clinical efficacy,without serious adverse reactions,it is worth promoting.
