解毒醒脑合剂联合针刺治疗脑出血后意识障碍疗效观察

Effect of Jiedu Xingnao Mixture and Acupuncture on Consciousness Disorder after Cerebral Hemorrhage

[目的]观察解毒醒脑合剂联合针刺治疗脑出血后意识障碍疗效。[方法]将70例住院患者按病志号抽签方法随机分为两组;降低颅内压、防止感染、纠正水和电解质紊乱,吸氧、保持呼吸道通畅、防止并发症。对照组35例针刺治疗。取内关、人中、三阴交、极泉、委中、尺泽等穴位。内关直刺0.5~1寸,人中向鼻中隔斜刺0.3~0.5寸,雀啄法,三阴交45度进针1~1.5寸,提插补法,极泉直刺1寸,提插泻法,尺泽直刺1寸,百会、四神聪平刺0.3~0.5寸;神门直刺0.3~0.5寸,提插泻法。治疗组35例解毒醒脑合剂,33 m L/次,3次/d,无法吞咽予以鼻饲;针刺治疗同对照组。连续治疗7 d为1疗程。观测临床表现、清醒时间、GCS评分、不良反应。治疗1疗程(7 d),判定疗效。[结果]清醒时间治疗组短于对照组(P<0.01)。GCS评分两组均有升高(P<0.01),治疗组改升高多对照组(P<0.05)。[结论]解毒醒脑合剂联合针刺治疗脑出血后意识障碍,疗效满意,无严重不良反应,值得推广。

[Objective]To observe the curative effect of Jiedu Xingnao Mixture combined with acupuncture on consciousness disorder after cerebral hemorrhage.[Method]Seventy inpatients were randomly divided into two groups by drawing lots according to the disease sign by random parallel control method,which included reducing intracranial pressure,preventing infection,correcting water and electrolyte disorders,inhaling oxygen,keeping respiratory tract unobstructed and preventing complications.35 cases in the control group were treated with acupuncture.Neiguan,Renzhong,Sanyinjiao,Jiquan,Weizhong,Chize and other acupoints were taken.Neiguan straight needle 0.5-1 inch,middle nasal septum oblique needle 0.3-0.5 inch,pecking method,Sanyinjiao 45 degree needle 1-1.5 inch,lifting interpolation method,Jiquan straight needle 1 inch,lifting and reducing method,Chize straight needle 1 inch,Baihui,Sishen Cong flat needle 0.3-0.5 inch;Shenmen straight needle 0.3-0.5 inch,lifting and reducing method.In the treatment group,35 cases of Jiedu Xingnao Mixture,33 m L/time,3 times/day,could not be swallowed for nasal feeding;acupuncture treatment was the same as the control group.Continuous treatment for 7 days is a course of treatment.Clinical manifestations,awakening time,GCS score and adverse reactions were observed.One course of treatment(7 days)was used to determine the curative effect.[Result]The awakening time in the treatment group was shorter than that in the control group(P<0.01).The GCS scores in both groups were increased(P<0.01),while those in the treatment group were increased more than those in the control group(P<0.05).[Conclusion]Jiedu Xingnao Mixture combined with acupuncture has satisfactory curative effect on consciousness disorder after cerebral hemorrhage,and has no serious adverse reactions.It is worth popularizing.
