分析血清降钙素原对不同种类的细菌引起脓毒症的早期鉴别诊断价值和临床应用 被引量:5

摘要 目的:探讨分析血清降钙素原对不同种类细菌引起脓毒症的早期鉴别诊断价值与临床应用.方法:选择自2017年8月~2018年8月我院收治的100例因革兰阴性菌、革兰阳性菌等不同种类细菌引起的脓毒症患者为观察组对象,比较分析血清降钙素原对不同种类细菌所致的脓毒症的诊断结果.结果:所有患者经血培养显示G-菌组检出55株致病菌,其中大肠埃希菌、肺炎克雷伯菌、鲍曼不动杆菌占比较高;而G+菌组检出45株致病菌,其中人葡萄球菌、表皮葡萄球菌、金黄色葡萄球菌占比较高.且G-菌组血清降钙素原水平相比较G+菌组更高,组间相比差异有统计学意义(P<0.05);而血清降钙素原识别G-菌的灵敏度为97.0%,其检测特异性为85.0%.结论:在对不同种类细菌引起的脓毒症患者可用血清降钙素原行早期鉴别诊断,其敏感性与特异性高,有助于为临床治疗工作的展开提供可靠依据. Objective: To explore the early differential diagnosis value and clinical application of serum procalcitonin in sepsis caused by different kinds of bacteria. Methods: From August 2017 to August 2018, 100 patients with sepsis caused by Gram-negative bacteria or Gram-positive bacteria, were selected as the observation group. The diagnostic results of serum procalcitonin for sepsis caused by different kinds of bacteria were compared and analyzed. Results: 55 bacteria were detected in G-bacteria group, including Escherichia coli, Klebsiella pneumoniae and Acinetobacter baumannii, while 45 bacteria were detected in Gram-positive bacteria group, including Staphylococcus hominis, Staphylococcus epidermidis and Staphylococcus aureus. The serum procalcitonin level of G-bacteria group was higher than that of Gram-positive bacteria group, and there was significant difference between groups (P<0.05), while the sensitivity of serum procalcitonin to identify G-bacteria was 97.0%, and the detection specificity was 85.0%. Conclusion: Serum calcitonin can be used for early differential diagnosis of sepsis caused by different kinds of bacteria (Gram-negative bacteria or Gram-positive bacteria). It has high sensitivity and specificity, and is helpful to provide reliable basis for clinical treatment.
