参麦注射液联合曲美他嗪和左卡尼汀治疗急性病毒性心肌炎的临床效果

Clinical effect of Shenmai injection combined with trimetazidine and levocarnitine on acute viral myocarditis

目的探讨参麦注射液联合曲美他嗪和左卡尼汀治疗急性病毒性心肌炎的临床效果。方法回顾性分析我院2017年8月至2018年8月收治的60例急性病毒性心肌炎患者的临床资料,根据治疗方法不同将患者分为常规组(n=30例,曲美他嗪+左卡尼汀)和联合组(n=30例,参麦注射液+曲美他嗪+左卡尼汀)。分析两组的临床疗效、主要临床症状改善时间、心功能恢复时间、用药时间、炎症因子水平及心功能NYHA分级改善情况。结果联合组的治疗总有效率为96.67%,显著高于常规组的80.00%,差异具有统计学意义(P<0.05)。联合组乏力、胸闷、心悸、心脏扩大改善时间均显著短于常规组,心功能恢复时间和用药时间均显著短于常规组,差异具有统计学意义(P<0.05)。治疗后,两组血清TNF-α和IL-6水平均显著降低,且联合组低于常规组,差异具有统计学意义(P<0.05)。治疗后,联合组的心功能NYHA分级明显优于常规组及治疗前,差异具有统计学意义(P<0.05)。结论参麦注射液联合曲美他嗪和左卡尼汀治疗急性病毒性心肌炎的效果显著,能够改善患者的临床症状,降低血清炎性因子水平,促进心功能的恢复。

Objective To investigate the clinical effect of Shenmai injection combined with trimetazidine and levocarnitine on acute viral myocarditis. Methods The clinical data of 60 patients with acute viral myocarditis admitted in our hospital from August 2017 to August 2018 were retrospectively analyzed. The patients were divided into routine group (n=30, trimetazidine + levocarnitine) and combined group (n=30, Shenmai injection + trimetazidine + levocarnitine). The clinical efficacy, improvement time of main clinical symptoms, recovery time of cardiac function, medication time, levels of inflammatory factors and NYHA grading improvement of cardiac function were analyzed. Results The total effective rate of treatment was 96.67% in the combined group, which was significantly higher than 80.00% in the routine group, and the difference was statistically significant (P<0.05). The improvement time of fatigue, chest tightness, palpitation and cardiac enlargement in the combined group were significantly shorter than those in the routine group, and the recovery time of cardiac function and medication time were significantly shorter than those in the routine group, and the differences were statistically significant (P<0.05). After treatment, the levels of serum TNF-α and IL-6 in both groups significantly decreased, and those in the combined group were lower than the routine group, and the differences were statistically significant (P<0.05). After treatment, the NYHA grading of cardiac function in the combined group was significantly better than that in the routine group and before treatment, and the differences were statistically significant (P<0.05). Conclusion Shenmai injection combined with trimetazidine and levocarnitine has a significant effect on acute viral myocarditis. It can improve the clinical symptoms of patients, reduce the level of serum inflammatory factors and promote the recovery of cardiac function.
