生精片联合他莫昔芬治疗勃起功能障碍合并少弱精子症研究

Treatment of mild-moderate erectile dysfunction complicated with oligospermia and asthenospermia by the combination of Shengjing tablets and tamoxifen

目的探讨针对合并勃起功能障碍的少弱精子症患者,采用生精片与他莫昔芬联合治疗的效果。方法随机选取前来清远市人民医院泌尿外科门诊就诊,存在不育症病史、精液常规检查提示少弱精子症、临床诊断为轻中度勃起功能障碍的120例患者为研究对象。将其随机分为生精片治疗组(n=37)、他莫昔芬治疗组(n=38)和双药联合治疗组(n=37)。观察治疗前后三组患者IIEF-5评分、精液常规和性激素检验结果等指标的变化。结果治疗前,三组患者基础特征、精液质量、性生活状况等方面比较,其差异均无统计学意义;治疗后,三组患者在性生活状况和精液质量方面均有了一定程度的好转。其中,双药联合治疗组患者在IIEF-5评分(20.40比16.30,P<0.001)、精子密度(21.08×10^6比12.99×10^6,P<0.001)和精子活力(36.29%比25.38%,P<0.001)、睾酮值(3.92 ng/ml比3.77 ng/ml,P=0.001)等多个方面均有了明显的改善。相对于单药治疗,双药联合治疗组患者在IIEF-5评分改善(4.10比3.16比1.55,P<0.001)和精子密度改善(8.09×10^6比7.66×10^6比7.39×10^6,P=0.002)方面效果最为突出。结论联合应用生精片和他莫昔芬可以有效改善合并轻中度勃起功能障碍的少弱精子症患者的精液质量,提高患者的性生活质量。

Objective To investigate the effects of treating mild-moderate erectile dysfunction complicated with oligospermia and asthenospermia by combination of Shengjing tablets and tamoxifen. Methods 120 patients who visited Department of Urology, Qingyuan People′s Hospital, with history of infertility, confirmed with oligospermia and asthenospermia by routine examination of semen and clinically diagnosed with mild-moderate erectile dysfunction were selected, and randomly divided into Shengjing tablets group(n=37), tamoxifen group(n=38) or combination treatment group(n=37). Results Before the treatment, there was no statistically significant difference among the 3 groups in baseline characteristics, semen quality and sexual life. After the treatment, there′s significant improvement of sexual life and semen quality in all the 3 groups. Particularly, the IIEF-5 score(20.40 vs 16.30, P<0.001), sperm density(21.08×10^6 vs 12.99×10^6, P<0.001), sperm motility(36.29% vs 25.38%, P<0.001) and serum testosterone value(3.92 ng/ml vs 3.77 ng/ml, P=0.001) were significantly increased in combination treatment group. Compared with other groups, the improvement of IIEF-5 score(4.10 vs 3.16 vs 1.55, P<0.001) and sperm density(8.09×10^6vs7.66×10^6vs7.39×10^6, P=0.002) were far more obvious in combination treatment group. Conclusions The combination of Shengjing tablets and tamoxifen is effective in improving the quality of semen and sexual life of patients with mild-moderate erectile dysfunction complicated with oligospermia and asthenospermia.
