摘要
目的:探讨胎儿期及婴儿期暴露于地震应激与促肾上腺皮质激素释放激素受体1(CRHR1)基因多态性对成年期个体睡眠质量的影响。方法:共纳入经历地震应激者556例,根据地震应激时点分为婴儿期暴露组(n=172)、胎儿期暴露组(n=179)及震后出生的对照组(n=205)。采用汉密顿焦虑(HAMA)、匹兹堡睡眠质量指数(PSQI)及聚合酶链式反应(PCR)分别测试3组人群睡眠质量、焦虑状态与CRHR1基因多态性。结果:胎儿期暴露组PSQI≥7分者的比例高于对照组(P <0.01);CRHR1的rs242924与rs7209436位点携带C等位基因的个体睡眠质量异常检出率高于未携带C等位基因个体(均P <0.05)。多因素logistic回归分析显示暴露于地震应激(OR=2.08)、rs7209436位点C等位基因表达(OR=1.98)以及焦虑状态(OR=2.65)对成年期个体睡眠质量异常发生存在风险。结论:胎儿期与婴儿期遭受地震应激、焦虑以及rs242924与rs7209436C等位基因高表达是成年期个体睡眠质量异常的危险因素。
Objective: To investigate the effects of fetal and infant exposure to earthquake stress and the polymorphism of corticotropin releasing hormone receptor 1(CRHR1) gene on sleep quality in adults.Methods: Totally556 individuals who experienced earthquake stress were selected and divided into 3 groups according to the different period of earthquake exposure,namely,infancy exposure groups(n = 172),fetal exposure group(n =179) and control group(n = 205).The Hamilton Anxiety Scale(HAMA),Pittsburgh sleep quality index(PSQI),and Polymerase Chain Reaction(PCR) were used to measure sleep quality,anxiety state and CRHR1 gene polymorphism respectively.Results: The proportion of patients with PSQI scores of ≥7 was higher in the fetal exposure group than in the control group(P < 0.01).Rs242924 and rs7209436 sites in CRHR1 showed that the detection rate of C allele in individuals with abnormal sleep quality was higher than those without the C allele(Ps< 0.05).Multivariate logistic regression analysis showed that exposure of earthquake stress(OR = 2.08),expression of C allele in rs7209436 site(OR = 1.98) and anxiety state(OR = 2.65) were the risk factors of abnormal sleep quality.Conclusions: Exposure to earthquake stress in fetal and infant,anxiety and high expression of C genotype in rs242924 and rs7209436 sites may be the risk factors of abnormal sleep quality in adulthood.
