Comparison of Application of MRI-DWI and CT Perfusion Imaging in Evaluating Tumor Activity After Interventional Therapy for Liver Cancer

摘要 目的观察核磁共振弥散加权成像(MRI-DWI)、CT灌注扫描(CTPI)在评估肝癌经导管动脉化疗栓塞术(TACE)术后肿瘤活性中的应用价值。方法对58例(69个病灶)肝癌TACE术后患者临床资料进行回顾性分析。患者均于术后3~5周接受MRIDWI、CTPI和数字减影血管造影(DSA)检查,以DSA为“金标准”标准,评估MRIDWI与CTPI对肝癌TACE术后活性病灶的诊断价值。结果MRI-DWI与CTPI诊断肝癌TACE术后活性病灶的灵敏度、特异度、准确度、阳性预测值、阴性预测值差异均无统计学意义(P>0.05);MRI-DWI与DSA诊断一致性高于CTPI。结论MRI-DWI、CTPI对肝癌患者TACE术后肿瘤活性均有良好评估作用,2种方法各有优劣,合理应用上述诊断措施对指导临床治疗有重要意义。 Objective To observe the application value of magnetic resonance imaging diffusion weighted imaging(MRI-DWI)and CT perfusion imaging(CTPI)in evaluating the tumor activity after transcatheter arterial chemoembolization(TACE)for liver cancer.Methods Clinical data of 58 cases(69 lesions)of patients with liver cancer after TACE were retrospectively analyzed.All patients were given MRI-DWI,CTPI and digital subtraction angiography(DSA)at 3 to 5 weeks after operation.DSA was used as the gold standard criteria to evaluate the diagnostic value of MRI-DWI and CTPI on active lesions after TACE for liver cancer.Results There were no statistically significant differences in the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of MRI-DWI and CTPI in diagnosing active lesions of liver cancer after TACE(P>0.05).The diagnostic consistency of MRI-DWI and DSA was high than that of CTPI.Conclusion MRI-DWI and CTPI both have good evaluation on tumor activity of patients with liver cancer after TACE.The two methods have their own advantages and disadvantages.Rational application of these diagnostic measures is of great significance in guiding clinical treatment.
