摘要 目的比较电子计算机断层扫描(CT)和磁共振成像(MRI)诊断强直性脊柱炎(AS)患者骶髂关节面下骨质侵蚀的价值。方法选取我院收治的75例AS患者,均行CT与MRI检查,比较2种诊断方式影像学检出效果、AS骶髂关节病变分级。结果 CT与MRI骶髂关节面下骨质硬化、关节间隙变宽与关节间隙变窄检出率比较无显著差异(P>0.05),MRI对骶髂关节面下骨质侵蚀(78.67%)、骨质囊变(61.33%)与软骨肿胀(33.33%)检出率明显高于CT64.00%、40.00%、16.00%(P<0.05);CT与MRI对AS骶髂关节病变0级、Ⅲ级与Ⅳ级诊断比较无明显差异(P>0.05),MRI对Ⅰ级(26.67%)与Ⅱ级(45.33%)检出率明显高于CT13.33%、29.33%(P<0.05)。结论相较于CT,MRI对AS患者骶髂关节病变的影像学检出效果更突出,能够有效检出骶髂关节面下骨质侵蚀、骨质囊变以及软骨肿胀等变化,为病变分级提供准确指导。 Objective To compare the value of computed tomography (CT) and magnetic resonance imaging (MRI) in the diagnosis of bone erosion under sacroiliac joint surface in patients with ankylosing spondylitis (AS).Methods 75 patients with AS admitted to the hospital underwent CT and MRI.The imaging detection effects and the classifications of AS sacroiliac joint lesions were compared by two diagnostic methods were compared. Results There was no significant difference in the detection rates of osteosclerosis, widening and narrowing of joint space under sacroiliac joint surface between CT and MRI (P>0.05).The detection rates for bone erosion (78.67%),bone cystic change (61.33%) and cartilage swelling (33.33%) under sacroiliac joint surface by MRI were significantly higher than those by CT (64.00%,40.00%,16.00%)(P<0.05).There was no significant difference in the diagnosis of AS sacroiliac joint lesions at grade 0,Ⅲ and Ⅳ between CT and MRI (P>0.05).The detection rates for AS sacroiliac joint lesions at grade Ⅰ(26.67%) and grade Ⅱ(45.33%) by MRI were significantly higher than those by CT (13.33%, 29.33%)(P<0.05).Conclusion Compared with CT,MRI is more prominent in the imaging detection effects of sacroiliac joint lesions in patients with AS.It can effectively detect the changes of bone erosion,bone cystic changes and cartilage swelling under sacroiliac joint surface,thus providing accurate guidance for lesion classifications.
