Effect of Damon Q self-ligating bracket + invisible aesthetic appliance on PLI and GI changes in patients with advanced orthodontic malocclusion

目的探讨DamonQ自锁托槽+隐适美矫治器对替牙中晚期牙性错颌畸形患者菌斑指数(PLI)、牙龈指数(GI)变化的影响.方法选取我院替牙中晚期牙性错颌畸形患者90例,依照治疗方案不同分为研究组(n=45)、常规组(n=45).常规组采用隐适美矫治器治疗,研究组采用DamonQ自锁托槽+隐适美矫治器治疗,统计比较两组治疗前、治疗3个月后牙周健康指标[牙龈指数(GI)、菌斑指数(PLI)、龈沟出血指数(SBI)]、龈沟液炎性因子[白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)]水平及治疗时间、复诊间隔时间.结果治疗3个月后研究组GI、PLI、SBI评分高于常规组(P<0.05);研究组治疗时间短于常规组,复诊间隔时间长于常规组(P<0.05);两组治疗3个月后IL-1β、TNF-α水平均高于治疗前(P<0.05),但组间对比无明显差异(P>0.05).结论DamonQ自锁托槽+隐适美矫治器对替牙中晚期牙性错颌畸形患者,可缩短其治疗时间,延长复诊间隔时间,但会引起PLI、GI、SBI及龈沟液炎性因子水平升高.

Objective To investigate the effect of Damon Q self-ligating bracket + invasive aesthetic appliance on plaque index (PLI) and gingival index (GI) in patients with advanced dental malocclusion. Methods 90 patients with advanced orthodontic malocclusion were selected and divided into study group (n=45) and routine group (n=45) according to different treatment options. The conventional group was treated with invisible aesthetic appliance. The study group was treated with Damon Q self-locking bracket + invisible aesthetic appliance. The periodontal health index [Gen's index (GI), before and after treatment for 3 months, was compared statistically. Plaque index (PLI), sulcus bleeding index (SBI), gingival fluid inflammatory factors [interleukin-1β(IL-1β), tumor necrosis factor-α(TNF-α)] levels and treatment time, follow-up interval time. Results After 3 months of treatment, the scores of GI, PLI and SBI in the study group were higher than those in the conventional group (P <0.05). The treatment time in the study group was shorter than that in the conventional group, and the interval between re-diagnosis was longer than that in the conventional group (P <0.05). The levels of IL- 1β and TNF-α were higher than those before treatment (P <0.05), but there was no significant difference between the groups (P >0.05). Conclusion Damon Q self-ligating bracket + invasive aesthetic appliance can shorten the treatment time and prolong the interval of rediagnosis in patients with advanced orthodontic malocclusion, but it will cause PLI, GI, SBI and gingival fluid inflammation. The level of factors is elevated.
