摘要 目的探讨负压创面引流技术(NPWT)在糖尿病足溃疡创面修复中的应用效果,以及科学护理对临床效果的影响.方法对2017年1月至2019年7月我院收治的28例Wagner分级为2~4级的糖尿病足溃疡患者实施一期彻底清创,应用NPWT技术,并加强负压引流管及创面管理,积极控制血糖水平,指导合理饮食,二期实施皮瓣移植修复,加强皮瓣护理,促进创面血液循环,控制创面感染,加速创面修复.结果28例患者中,9例经一期清创和NPWT技术治疗,创面较治疗前缩小70%以上,新鲜肉芽组织长出,后经加强换药和用药治疗,创面愈合;19例患者行二期皮瓣移植修复,最终痊愈;所有患者经相应护理干预,治疗期间血糖水平控制稳定,无引流管意外脱落、堵塞等意外事件发生,移植皮瓣生长良好,无严重并发症发生.结论NPWT技术可有效控制创面感染,为组织修复提供良好环境,促进新鲜肉芽生长;结合科学护理干预,可避免并发症及不良事件发生,提升治疗效果,加速患者创面修复. Objective To explore the application effect of negative pressure wound drainage technique (NPWT) in the repair of diabetic foot ulcer wounds and the effect of scientific nursing on clinical effects.Methods From January 2017 to July 2019, 28 patients with Wagner grade 2 to 4 diabetic foot ulcers were treated thoroughly in our hospital. The NPWT technique was applied and the negative pressure drainage tube and wound management were strengthened. Control blood sugar levels, guide reasonable diet, implement flap graft repair in the second phase, strengthen flap care, promote wound blood circulation, control wound infection, and accelerate wound repair.Results Of the 28 patients, 9 were treated with debridement and NPWT techniques. The wounds were reduced by more than 70% compared with before treatment. The fresh granulation tissue grew out, and the wounds were healed by intensive dressing and medication. The 19 patients underwent the second phase. The flaps were repaired and eventually healed;all patients were treated with corresponding nursing intervention, the blood sugar level was controlled stably during the treatment, and no accidents such as accidental shedding, clogging, etc. occurred, and the transplanted flaps grew well without serious complications.Conclusion NPWT technology can effectively control wound infection, provide a good environment for tissue repair, and promote fresh granulation growth. Combined with scientific nursing intervention, it can avoid complications and adverse events, improve treatment effect, and accelerate wound repair.
