XPERT在菌阴肺结核诊断价值的效果研究

摘要 目的探讨XPERT在菌阴性肺结核中的诊断价值。方法选取我院2018年1月-2019年5月期间收治的30例菌阴肺结核患者,排除结核病的其他肺部疾病患者30例,根据研究方法不同分为对照组与观察组。观察组行分枝杆菌培养、抗酸染色涂片及XPERT检测,对照组仅行XPERT检测,对分枝杆菌培养阳性患者行比例法药敏试验,对比2组患者XPERT检测阳性率及观察组三种检测方法检测阳性率。结果观察组XPERT检测阳性率较对照组高,差异有统计学意义(P<0.05);XPERT、分枝杆菌培养检测阳性率均较抗酸染色检测阳性率高,差异有统计学意义(P<0.05);22例分枝杆菌培养阳性标本中,经比例法检测出利福平耐药菌株2株,耐药率为9.09%,耐药标本患者中,XPERT法检出2株耐药菌株,检出率为100.00%。结论XPERT在菌阴肺结核中具有较高的诊断阳性率,诊断价值较高,且可有效诊断利福平耐药性,值得临床推广。 Objective To investigate the diagnostic value of XPERT in culture negative tuberculosis.Methods 30 patients with culture negative tuberculosis in the hospital from January 2018 to May 2019 were selected,and 30 patients with other pulmonary diseases who were excluded from tuberculosis were enrolled.All selected cases were divided into control group and observation group according to different research methods.Observation group was given mycobacterial culture,acid-fast smear and XPERT detection,while control group was only given XPERT detection.The patients with positive mycobacteria were given ratio method of drug sensitivity test.The positive rate of XPERT detection in two groups and the positive rate of three detection methods in observation group were compared.The diagnostic efficiency of XPERT detection was analyzed.Results The positive rate of XPERT detection in observation group was higher than that in control group(P<0.05).The positive rate of XPERT and mycobacterial culture was higher than that of acid-fast smear(P<0.05).Among 22 cases of positive samples of mycobacteria,2 strains of rifampicin resistant strain were detected by ratio method,and the drug resistance rate was 9.09%,2 strains of drug-resistant strain were detected by XPERT method among drug-resistant samples,and the detection rate was 100.00%.Conclusion XPERT in culture negative tuberculosis has higher positive diagnostic rate and higher diagnostic value,it can effectively diagnose the drug resistance of rifampicin,which is worthy of clinical promotion.
