摘要
目的评价全自动血细胞分析仪Sysmex XT-4000i体液模式对浆膜腔积液白细胞计数和分类的准确性和精密度.方法取20例临床患者浆膜腔积液标本,每个样本均分为2份,分别用于白细胞人工计数和仪器计数,分析其检验差异.比较不同形态学技术人员间人工计数和分类差异,同时比较仪器计数和分类重复性.选取白细胞数较高的浆膜腔积液标本,以生理盐水倍比稀释法稀释成不同浓度的梯度水平(1∶1、1∶2、1∶4、1∶8等),分别用仪器检测并做线性验证.结果20份浆膜腔积液的人工计数结果与仪器计数结果较一致(决定系数R^2=0.981,P=0.246).10名实验室人员检测同1份浆膜腔积液标本的白细胞计数结果变异系数(CV)为13%,仪器检测同1份浆膜腔积液标本10次的计数结果CV为3%.不同浓度梯度标本2次检测的平均值与理论值较一致(R^2=0.999,P=0.533),线性范围为10~2000个/微升,最低检测限可达10个/微升.结论Sysmex XT-4000i体液模式对浆膜腔积液白细胞计数的准确性、精密度均符合要求,线性范围和最低检测限可满足临床需求,白细胞分类的结果相对准确,在临床中具有应用价值.
Objective To evaluate the accuracy and precision of the automatic blood cell analyzer Sysmex XT-4000i body fluid mode in white blood cell count and classification of serous effusion.Methods Twenty specimens of serous effusion were divided into two parts,which were used for manual counting and automatic counting.The difference of the results reported by two methods was analyzed.The differences in manual counting and classification between different morphological technicians and the repeatability of automatic counts and classification were compared.The serous effusion specimens with higher white blood cell counts were selected and diluted to different gradient levels(1∶1,1∶2,1∶4,1∶8,etc)by physiological saline dilution method and detected by Sysmex XT-4000i body fluid mode for linear validation.Results The results of manual counting of 20serous effusions were consistent with the results of automatic counting(determination coefficient R^2=0.981, P=0.246).The coefficient variation (CV)of white blood cell counting among 10laboratory technicians was 13%,which of white blood cell couting detected by Sysmex XT-4000i for 10times was 3%.The average value of the two detections of different concentration gradient specimens was consistent with the theoretical value(R^2=0.999,P=0.533),the linear range was 10-2000 cells/μl, and the minimum detection limit was 10 cells/μl.Conclusion Sysmex XT-4000i body fluid mode may meet the requirements in the white blood cell count of serous effusion with good accuracy,precision and a good minimum detectable limitation.The result of white blood cell classification detected by Sysmex XT-4000i body fluid mode is relatively accurate,which has a clinical application value.
