嗜酸乳杆菌发酵产物对产ESBLs大肠埃希菌的抑制作用 被引量:1

Inhibitory effect of lactobacillus acidophilus fermentation products on extended-spectrum β-lactamases producing Escherichia coli

摘要 目的探讨嗜酸乳杆菌发酵产物对产超广谱β内酰胺酶(ESBLs)大肠埃希菌的抑制作用。方法对925例送检标本进行细菌分离与鉴定,分析产ESBLs大肠埃希菌耐药情况以及基因型分布情况,检测嗜酸乳杆菌发酵产物对产ESBLs大肠埃希菌的抑制作用。结果共分离出产ESBLs大肠埃希菌191株,主要分布于普外科(52例)、ICU(27例)、呼吸科(22例)和脑外科(18例)等科室。产ESBLs大肠埃希菌对大部分药物耐药率超过60%;其中,氨苄西林、氨曲南和阿米卡星耐药率达100%。表达CTX-M基因型的菌株83株,表达TEM基因型的菌株62株,同时表达CTX-M和TEM基因型的菌株21株,表达SHV基因型的菌株9株,同时表达CTX-M和SHV基因型的菌株12株,同时表达三种基因型的菌株4株。噬酸乳杆菌发酵产物对表达不同基因型的产ESBLs大肠埃希菌抑制效果强弱为CTX-M>SHV>TEM;其中,对表达CTX-M基因型的产ESBLs大肠埃希菌抑制效果强于表达TEM基因型的产ESBLs大肠埃希菌(P<0.05)。结论临床上,产ESBLs大肠埃希菌分布广泛,形势严峻,耐药率较高;噬酸乳杆菌对表达CTX-M基因型的产ESBLs大肠埃希菌有较强的抑制作用。 Objective To investigate the inhibitory effect of lactobacillus acidophilus fermentation products on extended-spectrum β-lactamases (ESBLs)producing Escherichia coli. Methods Isolation and identification of bacteria were carried out in 925 samples.The drug resistance and genotype distribution of ESBLs producing Escherichia coli were analyzed.The inhibitory effect of lactobacillus acidophilus fermentation products on ESBLs producing Escherichia coli was detected. Results A total of 191 strains of ESBLs producing Escherichia coli was isolated.ESBLs producing Escherichia coli distributed mainly in the departments of general surgery(52cases),ICU(27cases), respiratory diseases(22cases)and brain surgery(18cases).The resistance rate of ESBLs producing Escherichia coli was more than 60% to the majority of drugs,which was 100% to ampicillin, aztreonam and amikacin.CTX-M genotype was expressed in 83 strains,TEM genotype in 62strains, both of CTX-M and TEM genotypes in 21 strains,SHV genotype in 9 strains,both of CTX-M and SHV in 12 strains and three genotypes in 4strains.The inhibitory effect of lactobacillus acidophilus fermentation products on different genotypes of ESBLs producing Escherichia coli was CTX-M>SHV >TEM,which was higher in ESBLs producing Escherichia coli expressing CTX-M than that expressing TEM (P<0.05).Conclusion Clinically,ESBLs producing Escherichia coli are widely distributed,the situation is serious,and the drug resistance rate is high.Lactobacillus acidophilus has a strong inhibitory effect on ESBLs producing Escherichia coli expressing CTX-M genotype.
