摘要
目的:探讨依达拉奉治疗急性脑梗死(ACI)患者的效果及作用机理.方法:选取我院2016年3月~2017年12月收治的ACI患者80例,采用随机数字表法分为试验组和对照组各40例,两组患者均给予基础治疗,试验组同时给予依达拉奉治疗;对比两组患者治疗前后的神经功能缺损评分(NIHSS)、巴氏评分(Barthel)、血清脑源性神经营养因子(BDNF)、神经生长因子(NGF)、肿瘤坏因子-α(TNF-α)、C反应蛋白(CRP)、大脑中动脉收缩期峰值血流(PSV)、舒张末期血流速度(EDV)、平均血流速度(Vm)、搏动指数(PI).结果:治疗前,试验组和对照组患者的NIHSS评分、Barthel评分差异不具有统计学意义;治疗28天后,试验组的NIHSS评分低于对照组,Barthel评分高于对照组;治疗前,试验组和对照组患者的血清BDNF、NGF、TNF-α、CRP水平差异不具有统计学意义;治疗14天后,试验组的TNF-α、CRP水平低于对照组,血清BDNF、NGF高于对照组;治疗前,试验组和对照组患者的大脑中动脉PSV、EDV、Vm、PI差异不具有统计学意义;治疗14天后,试验组的大脑中动脉PSV、EDV、Vm、PI低于对照组.结论:依达拉奉治疗ACI患者能有效的调节血清BDNF、NGF、TNF-α、CRP水平,改善大脑中动脉血流速度,对于患者神经功能恢复具有重要作用.
Objective To investigate the effect and mechanism of edaravone in the treatment of patients with acute cerebral infarction (ACI). Methods Eighty patients with ACI admitted to our hospital from March 2016 to December 2017 were enrolled in the study. The randomized digital table method was divided into 40 groups in the experimental group and the control group. The two groups were given basic treatment. The experimental group was given edara at the same time. Treatment;comparison of neurological deficit scores (NIHSS), Barthel scores, serum brain-derived neurotrophic factor (BDNF), nerve growth factor (NGF), tumor-derived factor-α(TNF) before and after treatment -α), C-reactive protein (CRP), systolic peak blood flow (PSV), enddiastolic blood flow velocity (EDV), mean blood flow velocity (Vm), and pulsatility index (PI). Results Before treatment, there was no significant difference in NIHSS score and Barthel score between the experimental group and the control group. After 28 days of treatment, the NIHSS score of the experimental group was lower than that of the control group, and the Barthel score was higher than that of the control group. The control group;before treatment, the serum levels of BDNF, NGF, TNF-α and CRP in the experimental group and the control group were not statistically significant;after 14 days of treatment, the TNF- in the experimental group The levels of a and CRP were lower than those of the control group, serum BDNF and NGF were higher than the control group. Before treatment, the difference of PSV, EDV, Vm and PI in the middle cerebral artery of the experimental group and the control group was not the same. Statistically significant;after treatment for 14 days, the PSV, EDV, Vm, and PI in the middle cerebral artery of the experimental group were lower than those in the control group. Conclusion Edaravone in the treatment of ACI patients can effectively regulate serum BDNF, NGF, TNF-α, CRP levels, improve the blood flow velocity of the middle cerebral artery, and play an important role in the recovery of neurological function.
