摘要
目的观察磁共振黑血血栓成像(MRBTI)技术在脑静脉血栓(CVT)检测中的应用。方法对2例拟诊CVT的患者进行MRBTI检查,并分析其临床、影像学检查资料。结果病例1:怀孕39^+4周,围产期反复癫发作,MRBTI示上矢状窦前段及对应脑浅静脉血栓形成,TOF-MRV提示上矢状窦前段血栓。病例2:女57岁,因"突发头痛7d,加重伴左侧肢体无力1d"住院,MRBTI示上矢状窦及双侧额顶叶浅静脉、左侧横窦、乙状窦、颈内静脉血栓形成(部分亚急性期)。结论MRBTI能够准确检测CVT,并与周围组织、血管对比良好,有望作为一线检查手段。
Aim To observe a novel MR black-blood thrombus imaging technique ( MRBTI) for detection of cerebral venous thrombosis ( CVT).Methods MRBTI was performed on 2 patients with suspected CVT.The clinical and image data of 2 patients with proven CVT by conventional imaging techniques and MRBTI were analyzed.Results Patient 1 was a 39^+4-weeks-pregnant woman with a history of recurrent seizures in the peripartum period,MRBTI demonstrated hyperintense signal intensity in the anterior part of the superior sagittal sinus and the cortical veins,suggesting intraluminal thrombus formation. The thrombus was also confirmed on TOF-MRV.Patient 2 was a 57-year-old female,she was admitted into our department with headache and vomiting for 7 days,followed by weakness of left hemiplegia for a day. MRBTI demonstrated hyperintense signal intensity in the superior sagittal sinus,the left transverse,sigmoid sinus and jugular vein and the cortical veins,suggesting intraluminal fresh and sub-acute thrombus. Conclusion MRBTI can detect and isolate CVT from the surrounding tissues including lumen and wall with a high diagnostic accuracy,holding promise to serve as a first-line diagnostic imaging test.
