二维应变成像评估老年高血压合并阵发性房颤患者左心房心肌功能的价值

Application Value of Two-dimensional Strain Imaging in Evaluating Left Atrial Myocardial Function in Elderly Patients with Hypertension and Paroxysmal Atrial Fibrillation

[目的]探讨应用二维应变成像评估老年高血压合并阵发性房颤患者左房心肌功能的价值.[方法]选择2018年1~12月本院收治的86例老年高血压合并阵发性房颤患者,根据患者左心室舒张功能不全的超声心动图征象数目将其分为零征象组、单征象组及多征象组(征象≥2),比较不同组别患者的心功能及左房心肌应变及应变率.[结果]左心室舒张功能不全患者零征象组有 20 例(23 .3%),单征象组有 35 例(40 .7%),而多征象组有 31 例(36 .0%).零征象组患者LVEF高于单征象、多征象组,LVEDD 、LVEDV 、LAVI低于单征象、多征象组,且组间比较差异均有统计学意义( P <0 .05).零征象组患者房间隔中段应变及应变率及左房侧壁中段应变及应变率均高于单征象、多征象组,且组间比较差异均有统计学意义( P <0 .05).[结论]在高血压合并阵发性房颤患者中,随着左心室舒张功能不全的超声心动图征象数目增多,患者左心房心肌功能明显降低.

[Objective] To evaluate the value of two‐dimensional strain imaging in the assessment of left atrial myocardial function in elderly patients who had hypertension complicated with paroxysmal atrial fibrillation .[Methods]A total of 86 pa‐tients with hypertension plus paroxysmal atrial fibrillation who were admitted to our hospital from January to December 2018 were enrolled.Patients were divided into different groups according to the number of echocardiographic signs of left ventricular diastolic dysfunction :zero sign group ,single sign group and multiple signs group .Myocardial function and left atrial myocar‐dial strain in different groups of patients were compared .[Results]Based on the number of echocardiographic signs of left ven‐tricular diastolic dysfunction in patients ,there were 20 cases (23 .3%) in the zero sign group ,35 cases (40.7%) in the single sign group and 31 cases (36 .0%) in the multiple signs group .The results showed that LVEF of patients with the zero sign group was higher than that of the single sign and multiple signs group ,while LVEDD ,LVEDV and LAVI were lower than those of the single sign and multiple signs group .The differences were statistically significant ( P <0 .05).The strain and strain rate in the middle segment and the middle wall of the left atrial wall of the zero sign group were higher than those of the single sign and multiple signs group;the differences were statistically significant ( P <0 .05).[Conclusion]With the increase in the number of echocardiographic signs of left ventricular diastolic dysfunction ,the left atrial myocardial function in hyperten‐sion patients complicated with paroxysmal atrial fibrillation was significantly reduced .
