不同麻醉药物用于剖宫产手术腰‐硬联合麻醉的临床效果比较

Comparison of Clinical Effects of Different Anesthetics on Spinal-epidural Anesthesia in cesarean Section

[目的]探讨比较罗哌卡因与布比卡因用于剖宫产手术腰硬联合麻醉的效果.[方法]本院220例行剖宫产孕妇,随机分为两组,各110例;观察组采用罗哌卡因麻醉.对照组采用布比卡因麻醉.比较两组孕妇的麻醉效果及不良反应发生情况.[结果]两组孕妇麻醉后平均动脉压、心率与麻醉前相比均降低,且差异有显著性( P <0 .05).观察组麻醉后5 min的平均动脉压、心率高于对照组( P <0 .05).观察组不良反应发生率、术中低血压发生率均低于对照组(P <0 .05),观察组麻醉后感觉运动阻滞情况均优于对照组(P <0 .05).[结论]剖宫产手术腰硬联合麻醉中使用罗哌卡因、布比卡因均具有较好的麻醉效果,但罗哌卡因对孕妇的影响较小,不良反应发生概率较低且轻,值得推广应用.

[Objective]To explore the anesthetic effect of ropivacaine and bupivacaine on spinal-epidural anesthesia in cesarean section .[M ethods]A total of 220 cases of cesarean section pregnant women admitted to our hospital were randomly divided into two groups ,with 110 cases in each group .T he observation group was anesthetized with ropivacaine .the control group was anesthetized with bupivacaine .T he anesthesia effect and adverse reactions were observed in the two groups .[Results]T he average arterial pressure and heart rate of the two groups after anesthesia were lower than those before anesthesia ( P <0 .05).T he mean arterial pressure and heart rate at 5 minutes after anesthesia in the observation group were higher than those in the control group ( P <0 .05).T he incidence of adverse reactions and intraoperative hypotension in the observation group were lower than those in the control group ( P <0 .05).T he sensorimotor block in the observation group was better than that in the control group after anesthesia ( P <0 .05).[Conclusion]Both ropivacaine and bupivacaine are effective in spinal‐epidural anesthesia for cesarean section .However ,ropivacaine has less effect on pregnant women ,and the incidence of adverse reactions are mild .So it is worthy of popularization .
