基于QOL评价的自我管理模式在HIV(+)MSM中的实证研究

An empirical study of self-management model based on QOL evaluation in HIV-infected MSM

目的利用QOL综合指标评价自我管理模式在感染HIV的MSM人群中的实践效果,为HIV/AIDS防治与管理提供数据支撑和参考依据。方法根据纳入排除标准选取上海市420名MSM人群随机分为干预组和对照组。干预组为自我管理模式组,建立MSM自我管理小组,在CDSM理论基础上以小组为单位开展健康教育课程和同伴教育。对照组为常规随访管理模式组,即以疾控中心为主体开展的医学随访等相关活动。干预1年后开展QOL评估调查,采用SPSS软件对数据进行统计分析。结果自我管理小组干预前后在健康感受(60.5~64.9分)、角色功能(92.1~97.1分)、生活质量(70.4~79.8分)、健康转换(69.4~74.9分)、心理健康总得分(51.1~59.9分)5个指标的得分上有不同程度的提高。差异有统计学意义(P<0.05)。结论自我管理模式可以帮助HIV(+)MSM改善生存质量,值得进一步推广和验证。

Objective To evaluate the effects of self-management model in HIV-infected MSM population based QOL comprehensive indicators,and to provide data support and reference for HIV/AIDS prevention and management.Methods According to the inclusion and exclusion criteria,420 MSM in Shanghai were randomly divided into intervention group and control group.The intervention group was a self-management model group in which the MSM self-management team was established.Based on the CDSM theory,the health education curriculum and peer education were carried out.The control group was the routine follow-up management mode group,which was related to medical follow-up activities organized by the CDC.The QOL assessment survey was conducted after 1 year of intervention,and the data was statistically analyzed using SPSS software.Results QOL evaluation showed different degrees of improvement in health experience(60.5 to 64.9 points),role function(92.1 to 97.1 points),quality of life(70.4 to 79.8 points),health conversion(69.4 to 74.9 points),and total mental health score(51.1 to 59.9 points)in the self-management group before and after intervention.The difference was statistically significant(P<0.05).Conclusion The self-management model can help HIV(+)MSM improve the quality of life and deserve further promotion and validation.
