摘要
目的评估2004—2018年荆州市血吸虫病综合治理效果,为实现消除血吸虫病目标提供依据。方法收集2004—2018年荆州市血吸虫病综合治理项目年报资料,分析比较血吸虫病疫情防控效果,进行血防效益评价。结果全市人群感染率由2004年9.34%降至2016年0,2010年以来连续9年未发现本地感染的急性血吸虫病病例。耕牛感染率由2004年8.05%降至2013年0。2012年以来连续7年未查到感染性钉螺。人畜感染率平均每村每降1个百分点投入综合治理经费,疫情控制阶段分别为11.97万元、13.77万元,传播控制阶段分别为75.62万元、71.64万元;传播阻断阶段人群为270.61万元,家畜巩固经费为175.89万元。结论实施以传染源控制为主的综合治理措施,是当前消除血吸虫病危害有效措施,进一步巩固血防成果,仍需强化家畜传染源的防控,但巩固成果投入综合治理成本较高。
Objective To evaluate the comprehensive management effect of schistosomiasis in Jingzhou City from 2004 to 2018,and provide a basis for achieving the goal of eliminating schistosomiasis.Methods The annual report data of Jingzhou City Schistosomiasis Comprehensive Management Project from 2004 to 2018 was collected.The effects of prevention and control of schistosomiasis were analyzed and evaluated.Results The infection rate of the total population in the city dropped from 9.34%in 2004 to 0%in 2016.No cases of acute schistosomiasis had been detected in local infection for 9 consecutive years since 2010.The infection rate of cattle was reduced from 8.05%in 2004 to 0%in 2013.Infectious snails had not been detected for 7 consecutive years since 2012.For every 1 percentage point drop in the infection rate of humans and animals per village,it was necessary to invest in the corresponding comprehensive management funds at different stages of the management of schistosomiasis.At the epidemic control stage funds were 119 700 yuan and 137 700 yuan,respectively.At the transmission control stage,funds were 756 200 yuan and 716 400 yuan,respectively,while at the stage of transmission blocking,it required 2 706 100 yuan for the human crowd,and 1 758 900 yuan for the livestock consolidation.Conclusion At present the implementation of comprehensive control measures based on infection source control was an effective measure to eliminate schistosomiasis.To further consolidate the results of schistosomiasis prevention,it is still necessary to strengthen the prevention and control of livestock infection sources,but the cost for the comprehensive treatment for consolidating achievements is relatively high.
