41例腹部创伤为主多发伤手术治疗的临床研究

Clinical Study on 41 Cases of Abdominal Trauma with Multiple Injuries

目的探讨腹部创伤为主多发伤手术治疗的临床效果,分析治疗方案,总结治疗经验,为提高该类疾病救治水平、降低死亡率提供依据参考。方法方便选取2016年1月—2018年12月该院急诊收治的41例腹部创伤为主多发伤患者为研究对象,依据症状体征、影像学检查、腹腔穿刺等明确患者损伤部位与伤害程度,以此为据制定个体化手术治疗方案,观察临床治疗效果,统计并发症。结果36例患者手术治愈,1例手术好转后转院,4例死亡,治疗有效率90.24%。术后,切口开裂2例、肺不张2例、胃瘫1例、胆漏1例,并发症发生率14.63%。结论外科手术是腹部创伤为主多发伤的有效治疗手段,根据患者病情个体化制定救治方案,合理安排手术,能够获得理想疗效,降低患者死亡风险,提高救治成功率。

Objective To investigate the clinical effect of surgical treatment of multiple traumatic abdominal trauma,analyze the treatment plan,summarize the treatment experience,and provide reference for improving the treatment level and mortality of such diseases.Methods Forty-one patients with multiple traumatic injuries of the abdomen in our hospital from January 2016 to December 2018 were convenient selected and enrolled in the study.According to the symptoms and signs,imaging examination,abdominal puncture,etc.,the injury site and injury degree were determined.This is based on the development of individualized surgical treatment programs,observation of clinical treatment effects,statistical complications.Results Thirty-six patients were cured by surgery.One patient was transferred to hospital after surgery,and 4 patients died.The effective rate of treatment was 90.24%.After operation,2 cases of incision were cleft,2 cases of atelectasis,1 case of gastric fistula,and 1 case of bile leakage.The complication rate was 14.63%.Conclusion Surgical operation is an effective treatment for multiple traumatic injuries in abdominal abdomen.According to the patient's condition,individualized treatment plan and reasonable arrangement of surgery can achieve ideal curative effect,reduce the risk of death and improve the success rate of treatment.
