摘要
目的探讨原发性肝细胞癌(hepatocellular carcinoma,HCC)组织中组蛋白甲基转移酶(SET and MYND domain-containing protein 2,SMYD2)表达水平与临床病理特征和预后的关系。方法回顾性选择2012年1月至2012年12月病理科保存的120例HCC组织标本作为研究对象。另选择同一患者癌旁正常组织(距离肿瘤组织>1.5 cm)作为对照。比较HCC组织及癌旁正常组织中SMYD2 mRNA相对表达量的差异。分析SMYD2表达水平与临床病理特征及预后的关系。应用单因素、多因素Cox回归分析影响HCC患者预后的危险因素。结果HCC组织中SMYD2 mRNA相对表达量为(5.23±1.18),高于癌旁正常组织中的(2.65±0.64),差异有统计学意义(P<0.05)。HCC组织中SMYD2表达水平与性别、年龄、吸烟史、嗜酒史、甲胎蛋白、HBV、HCV、肝硬化、肿瘤数量、远处转移无关(P>0.05),与分化程度、TNM分期、肿瘤直径、血管浸润有关(P<0.05)。单因素、多因素Cox回归分析结果显示,血管浸润、SMYD2阳性表达是影响HCC患者预后不良的危险因素(P<0.05)。Kaplan-Meier法生存曲线结果显示,HCC组织中SMYD2阴性表达组患者5年总生存率高于阳性表达组,差异有统计学意义(P<0.05)。结论肿瘤组织中SMYD2阳性表达不利于HCC患者预后。组织中SMYD2表达水平可作为预测HCC患者预后的重要指标。
Objective To investigate the expression of SET and MYND domain-containing protein 2(SMYD2)in hepatocellular carcinoma(HCC)and its correlation with clinicopathological features as well as the prognosis of patients with HCC.Methods A total of 120 tissue specimens coming from the patients with colorectal cancer wh owere treated in our hospital from January 2012 to December 2012 were enrolled as experimental group,at the same time,the normal tissues adjacent to the tumor coming from the same patient(distance from the tumor tissue>1.5 cm)were enrolled as control group.The relative expression levels of SMYD2 in HCC tissues and adjacent normal tissues were observed and compared between the two groups.Moreover the relationship between the expression of SMYD2 and clinicopathological features as well as patient’s prognosis was analyzed.In addition the univariate and multivariate Cox regression were used to analyze the risk factors affecting the prognosis of patients with HCC.Results The relative expression levels of SMYD2 mRNA in experimental group were 5.23±1.18,which were significantly higher than those(2.65±0.64)in control group(P<0.05).The expression levels of SMYD2 in HCC tissues were not related with sex,age,smoking history,alcohol addiction history,alpha-fetoprotein,HBV,HCV,cirrhosis,number of tumors and distant metastasis(P>0.05),however,which were correlated to differentiation degree of tumor,TNM stage,tumor diameter and vascular invasion(P<0.05).The u nivariate and multivariate Cox regression analysis showed that vascular infiltration and positive expression of SMYD2 were the risk factors resulting in poor prognosis of patients with HCC(P<0.05).Kaplan-Meier survival curve showed that the 5-year overall survival rate in SMYD2 negative expression group was significantly higher than that in SMYD2 positive expression group(P<0.05).Conclusion The positive expression of SMYD2 in tumors is unfavourable to the prognosis of patients with HCC,and the expression of SMYD2 in tissues can be regarded as important indexes to predict the prognosis of patients with HCC.
