急性脑出血患者血清CCCK-18水平检测及意义

Detection and significance of serum CCCK-18 level in acute cerebral hemorrhage patients

目的探讨细胞角蛋白 18 裂解片段( CCCK-18)在急性脑出血患者病情判断中的价值。方法选取陆军军医大学第二附属医院 2013 年 6 月至 2017 年 6 月 103 例急性脑出血患者,入院后按照美国国立卫生研究院卒中量表评分( NIHSS)分为 3组:高评分组( NIHSS≥15 分,25 例),中评分组( 6 分<NIHSS <15 分,40 例),低评分组( NIHSS≤6 分,38 例);另选取 30 名健康体检者作为健康对照组。检测脑出血患者颅内出血量,并运用酶联免疫吸附法( ELISA 法)检测所有纳入对象血清 CCCK-18 含量;进一步分析血清 CCCK-18 含量与 NIHSS 和颅内出血量的相关性。结果急性脑出血患者血清 CCCK-18 水平明显高于健康对照组,NIHSS 高评分组血清 CCCK-18 水平和颅内出血量显著高于中评分组,且中评分组高于低评分组,以上差异均有统计学意义( P <0. 05);急性脑出血患者血清 CCCK-18 水平与 NIHSS 呈明显正相关( r =0. 61,P <0. 05),与颅内出血量呈明显正相关( r = 0. 57,P < 0. 05)。结论急性脑出血患者血清 CCCK-18 水平明显增高,可以作为评估患者病情严重程度和预后恢复情况的生物学标志物。

Objective To investigate the value of serum caspase-cleaved Cytokeratin 18 ( CCCK-18) in evaluating the severity of acute cerebral hemorrhage. Methods This study comprised of 103 acute cerebral hemorrhage patients and 30 healthy control subjects. According to the National Institutes of Health Stroke Scale ( NIHSS),the 103 patients with acute cerebral hemorrhage were divided into three groups: high score group ( NIHSS≥15,n =25),median score group ( 6 < NIHSS <15,n =40),low score group ( NIHSS≤6,n =30). Hematoma volumes in patients was detected,and serum CCCK-18 content was detected by enzyme linked immunosorbent assay ( ELISA). The correlations between CCCK-18 and NIHSS and hematoma volumes were performed. Results The level of serum CCCK-18 were higher in acute cerebral hemorrhage patients compared with the healthy control group. The serum CCCK-18 levels and hematoma volumes were significantly high- er in high score group compared with the median score group,which were also higher than that in low score group ( P <0. 05). The correlation analyzes showed that CCCK-18 levels were significantly positively correlated to NIHSS score ( r = 0. 61,P < 0. 05) and hematoma vol- umes ( r =0. 57,P <0. 05) in acute cerebral hemorrhage patients. Conclusion The elevated serum CCCK-18 level can be a potential predictive biomarker to evaluate the severity and prognosis in acute cerebral hemorrhage patients.
