脑卒中后吞咽功能障碍的中医优化康复护理干预作用研究

Study on the Effect of Optimized Rehabilitation TCM Nursing Program for Stroke Patients with Dysphagia

目的观察中医优化康复护理方案在脑卒中后吞咽功能障碍患者康复中的应用效果。方法选取我院住院行康复治疗的脑卒中后吞咽功能障碍患者126例,随机分为观察组(72例)和对照组(54例)。观察组实施中医优化康复护理方案,对照组实施常规护理方案。以洼田饮水试验和腾岛一郎吞咽障碍试验进行疗效评价,并调查生存生活质量和护理满意度。结果治疗后,洼田饮水试验表明观察组的总有效率明显高于对照组(P<0.05),腾岛一郎吞咽障碍试验表明观察组患者吞咽功能明显改善;与治疗前相比,在生理功能(PF)、躯体疼痛(BP)、活力(VT)、社会功能(SF)和精神健康(MH)5个维度均有明显好转,但在生理职能(RP)、总体健康(GH)以及情感职能(RE)方面则无明显变化(P>0.05),而随访1个月末,观察组RP、GH 以及RE维度上均有明显的好转(P<0.05),且观察组的患者满意度明显高于对照组(P<0.05)。结论中医优化康复护理方案实施能改善脑卒中患者的吞咽功能,提高其生存生活质量,值得临床推广。

Objective To observe the effect of optimized rehabilitation nursing program of traditional Chinese medicine (TCM) for stroke patients with dysphagia. Methods 126 cases of patients with stroke were randomly divided into observation group (n=72) and control group (n=54). Control group was given routine nursing,while observation group received optimized rehabilitation nursing program on the basis of control group. The therapeutic effect was evaluated with Kubota water swallow test and Fujishima Ichiro swallowing disorder evaluation test,and the quality of life and nursing satisfaction degree were investigated. Results The results of water swallow test showed that whose overall response rate of observation group was higher than control group (P<0.05),and Fujishima Ichiro swallowing disorder evaluation test further indicated that the swallowing function of observation group was improved obviously. After treatment,PF,BP,VT,SF and MH five aspects were significantly improved (P<0.05),but RP,GH and RE three aspects weren’t changed obviously (P>0.05),while which were significantly improved at the end of one month of follow-up(P<0.05),and nursing satisfaction degree of observation group was superior to control group (P<0.05). Conclusion The optimized rehabilitation nursing program may improve the swallowing function of the stroke patients with dysphagia,enhance their quality of life,which is worthy of chinical promotion.
