晚期癌症患者家属病情告知态度调查

Attitude of Family Members on Truth Telling for Advanced Cancer Patients

目的了解晚期癌症患者家属对病情告知的态度,以及晚期癌症患者病情知晓情况与极端想法、行为的相关性。方法运用自制的病情告知情况调查表以及极端想法或行为调查表,对湖南省肿瘤医院2018年6-10月接受宁养服务的113位晚期癌症患者家属进行问卷调查。结果发现66.4%(75/113)的家属曾经对患者隐瞒病情,主要是担心导致患者心理压力大,以及害怕患者出现极端想法或行为。21.2%(24/113)的患者曾有放弃治疗的行为,38.9%(44/113)的患者提出目前活着没意思,想死,7.1%(8/113)人曾有自杀行为。而通过Logistic回归发现患者的这些极端想法或行为跟其病情知晓情况并无相关。结论因此如何进行妥善病情告知及做好情感支持,才是患者家属需要积极考虑的。同时在癌症患者的诊治过程中,肿瘤心理专业人员的及时介入,为患者和家属提供支持也尤为重要。

Objective To explore the attitude of family members of advanced cancer patients to telling the truth, and the correlation between cancer disclosure and the extreme thoughts or behaviors of advanced cancer patients. Methods From June to October 2018, 113 family members of advanced cancer patient in Hu’nan Cancer Hospital were investigated by self-made questionnaire, aiming to explore the cancer disclosure and extreme thoughts or behaviors. Results It revealed that 66.4%(75/113) of the family members had concealed diagnosis or prognosis from patients. Mainly worry about stress, and fear of thoughts or behaviors, such as suicide or giving up treatment. In this study, 21.2%(24/113)of the patients had given up treatment, 38.9%(44/113) of the patients mentioned they wanted to die, and 7.1%(8/113)had suicidal behavior. Logistic regression showed that these thoughts or behaviors were not related to their knowledge of disease. Conclusion Therefore, how to properly inform the disease and do a good job of emotional support, is the patient's family need to actively consider. At the same time, in the process of diagnosis and treatment of cancer patients, the timely intervention of psycho-oncology is also particularly important.
