摘要
目的分析小剂量盐酸罗哌卡因腰硬联合麻醉在剖宫产手术中的应用。方法选择2017年12月-2018年3月的入住我院的剖宫产手术患者66例,随机分为两组各33例,所有患者均行药硬联合麻醉分娩,参照组采用大剂量盐酸罗哌卡因,研究组则使用小剂量盐酸罗哌卡因。治疗后,比较两组产妇的麻醉阻滞效果、运动阻滞起效时间、感觉阻滞起效时间、Bromage评分以及不同时间点的临床指标。结果研究组麻醉阻滞效果、感觉阻滞起效时间、运动阻滞起效时间及Bromage评分低于参照组,差异具有统计学意义(P<0.05);麻醉后15min,研究组与参照组的心率、血氧饱和度、动脉压无显著差异(P>0.05);胎儿娩出后与剖宫产术后研究组和参照组的血氧饱和度、心率、动脉压相比,差异有统计学意义(P<0.05)。结论在剖宫产手术中,使用盐酸罗哌卡因腰硬(小剂量)联合麻醉后的临床疗效较好。
Objective To analyze the application of low-dose ropivacaine hydrochloride combined spinal-epidural anesthesia in cesarean section.Methods 66 cases of cesarean section patients admitted to our hospital from December 2017 to March 2018 were randomly divided into two groups, 33 cases in each group. All patients were given combined epidural anesthesia for delivery. The control group was given high dose ropivacaine hydrochloride, while the study group was given low dose ropivacaine hydrochloride. After treatment, the effects of anesthesia, onset time of motor block, onset time of sensory block, Bromage score and hemodynamics (heart rate, oxygen saturation, arterial pressure of the two groups were compared. Results The effect of anesthesia, onset time of sensory block, onset time of motor block and Bromage score in the study group were lower than those in the control group, with statistical significance (P < 0.05);15 minutes after anesthesia, there was no significant difference in heart rate, oxygen saturation and arterial pressure between the study group and the control group (P > 0.05);There were significant differences in oxygen saturation, heart rate and arterial pressure between the study group and the control group (P < 0.05). Conclusion Ropivacaine hydrochloride combined with spinal epidural anesthesia is effective in cesarean section.
