腹腔镜手术对早期宫颈癌治疗效果及对免疫水平及术后并发症影响

Effect of laparoscopic surgery for treating women with early cervical cancer, and its influence on immune level and postoperative complications of these women

目的:探究腹腔镜根治术对早期宫颈癌治疗效果及对免疫水平和术后并发症影响。方法:回顾性分析2016年2月—2018年5月本院宫颈癌行广泛子宫切除术87例临床资料,依据手术方式不同分为经腹组41例及腹腔镜组46例,比较分析两组术中情况、术前术后免疫指标及术后并发症。结果:腹腔镜组手术时间、淋巴结清扫数目均高于经腹组,而出血量低于经腹组(P<0.05)。术前1d,两组CD4^+、CD8^+、CD4^+/CD8^+比较无差异(P>0.05);术后3d,两组CD4^+、CD4^+/CD8^+均降低,CD8^+均上升,且腹腔镜组变化较经腹组高(P<0.05)。两组并发症发生率比较无差异(P>0.05)。结论:治疗早期宫颈癌,腹腔镜手术较经腹手术影响小,可减轻术后对患者的免疫抑制作用,安全性相似。

Objective: To investigate the therapeutic effect of laparoscopic surgery for treating women with early cervical cancer, and to study its influence on immune level and postoperative complications of these women. Methods: A retrospective analysis was conducted on 87 women with early cervical cancers who had experienced cervical extensive hysterectomy from February 2016 to May 2018. The included women were divided into group A (41 women experienced transabdominal cervical hysterectomy) and group B (46 women experienced laparoscopic cervical hysterectomy). Intraoperative conditions, levels of preoperative and postoperative immune indicators, and postoperative complications rate of women were compared between the two groups. Results: The operation time and the number of lymph node dissection of women in group B were significant higher than those of women in group A, but the amount of bleeding during operation was significant lower than that of women in group A ( P <0.05). There were no significant difference in number of CD4^+ and CD8^+, and value of CD4^+/CD8+ of women 1d before operation between the two groups ( P >0.05). At 3 days after operation, the number of CD4^+ and value of CD4^+/CD8^+ of women in the two groups decreased significantly, and those of women in group B were significant lower than those of women in group A ( P <0.05). There was no significant difference in the incidence of postoperative complications of women between the two groups ( P >0.05). Conclusion: Comparing to that of transabdominal surgery, laparoscopic cervical hysterectomy for treating women with early cervical cancer has less immunosuppressive effect on postoperative women, but the safety is similar.
