布地奈德混悬液和异丙托溴氨溶液雾化吸入治疗AECOPD疗效及对血清PCT、CRP、IL-6水平的影响 被引量:2

摘要 目的:探讨布地奈德混悬液和异丙托溴氨溶液雾化吸入治疗慢性阻塞性肺疾病急性加重期(AECOPD)疗效及对血清PCT、CRP、IL-6水平的影响。方法:选取我院2017年1月至2018年12月收治的AECOPD患者60例,按随机抽签法分为观察组(n=30)和对照组(n=30),对照组采取常规治疗,观察组在对照组基础上给予布地奈德混悬液和异丙托溴氨溶液雾化吸入治疗,观察两组治疗7d的临床疗效,比较两组治疗前后肺功能、血气分析指标及炎症因子PCT、CRP、IL-6水平和不良反应。结果:观察组治疗总有效率为96.67%,显著高于对照组73.33%(P<0.05);治疗后两组FEV1、FVC、FEV1/FVC、PEF及PaO2、SaO2、pH均较治疗前明显提高,PaCO2、降钙素原(PCT)、C-反应蛋白(CRP)、白细胞介素-6(IL-6)水平均较治疗前显著降低,且观察组均显著优于对照组(均P<0.05);两组不良反应比较无显著性差异(P>0.05)。结论:布地奈德混悬液和异丙托溴氨溶液雾化吸入治疗AECOPD能有效降低炎症因子PCT、CRP、IL-6水平,改善肺功能及血气指标,提高临床疗效,且无明显不良反应。 Objective: To investigate the curative effects of aerosol inhalation with budesonide suspension and ipratropium bromide solution on acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its influences on serum PCT,CRP and IL-6 levels.Methods: 60 patients with AECOPD admitted to the hospital from January 2017 to December 2018 were enrolled.They were divided into observation group(n=30)and control group(n=30)by sortition randomization method.The control group was given routine treatment.On this basis,the observation group was given aerosol inhalation with budesonide suspension and ipratropium bromide solution.The clinical curative effects at 7d after treatment of the two groups were observed. The lung function,blood gas analysis indexes,levels of inflammatory factors such as PCT,CRP and IL-6,adverse reactions before and after treatment were compared between the two groups.Results: The total response rate of the observation group was significantly higher than that of the control group(96.67% vs 73.33%)(P<0.05).After treatment,the FEV1,FVC,FEV1/FVC,PEF,PaO2,SaO2 and pH in both groups were significantly increased,while the levels of PaCO2,procalcitonin(PCT),C-reactive protein(CRP)and interleukin-6(IL-6)were significantly decreased.The observation group was significantly better than the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P >0.05).Conclusion: Aerosol inhalation with budesonide suspension and ipratropium bromide solution can effectively reduce the levels of inflammatory factors such as PCT,CRP and IL-6 for AECOPD,improve lung function and blood gas indexes,improve clinical curative effect,without obvious side effects.
