专科联盟建设中的伦理问题及对策思考

Thoughts on the Ethical Problems and Countermeasures in the Construction of Specialist Alliance

专科联盟作为四种常见的医联体模式之一,不仅促进了联盟内诊疗水平的提高,还使患者受益。为阐明专科联盟在建设过程中面临的诸多伦理问题,从规章制度不健全、运行模式不科学、三医机制存冲突、伦理意识缺失四个方面对专科联盟的主要伦理问题进行分析,在此基础上从规范顶层制度设计、重视伦理导向作用、健全信息网络三个方面提出应对措施。专科联盟运行中的伦理问题较为严重,应当予以重视。解决专科联盟建设工作中面临的主要伦理问题,有利于整合医疗卫生资源,增加患者的受益并缓解日益严峻的医患关系。

As one of the four common medical alliances, specialist alliance not only promotes the level of diagnosis and treatment within the alliance, but also brings benefits to patients. This paper analyzes the problems from four aspects including the lack of necessary rules and regulations, inadequate scientific operating mode, the conflict in the health system reform and the lack of ethical awareness. On this basis, some countermeasures are proposed from three aspects including normalizing the top institutional design, attaching importance to the ethical orientation and establishing perfect information network system. The ethical problems in the operation of specialist alliance are serious and should be paid attention to. Solving the main ethical problems in the construction of specialist alliance is conducive to integrating medical and health care resources, increasing the benefits of patients and alleviating the increasingly intense doctor-patient relationship.
