持续质量改进对提高尿毒症腹膜透析患者营养状况的效果分析

Analysis on effects of continuous quality improvement on improving nutritional status of uremia patients with peritoneal dialysis

目的研究分析在尿毒症腹膜透析患者中实施持续质量改进后对腹膜透析患者营养状况的改善作用。方法选取我院2016年6月~2017年6月收治的腹膜透析尿毒症患者66例作为本次研究对象,并采用随机数字表法分为观察组和对照组,两组患者均存在营养不良的症状,对照组采用常规治疗方法,观察组采用持续质量改进管理治疗法,每隔3个月对两组患者的营养状况变化进行评估,持续一年。结果护理一年后观察组患者营养指标中三头肌皮肤皱褶厚度、血清白蛋白指标、血清前白蛋白指标、Kt/V等指标均显著高于对照组患者,差异有统计学意义(P<0.05);观察组护理前后SGA分数下降幅度明显大于对照组,差异有统计学意义(P<0.05)。结论临床上通过持续质量改进对腹膜透析尿毒症患者进行管理,能有效改善患者的营养状况以及长期预后,有助于提高患者的生活质量,值得推广。

Objective To study and analyze the effect of continuous quality improvement for uremia patients undergoing peritoneal dialysis on improving nutritional status. Methods 66 uremia patients with peritoneal dialysis who were admitted and treated in our hospital from June 2016 and June 2017 were selected as research objects.According to the random number table method,they were randomly divided into the observation group and the control group.Patients in both groups had malnutrition symptoms.The control group was treated with routine treatment methods while the observation group was treated with continuous quality improvement.The nutritional status of the two groups was assessed every three months for one year. Results After nursing,the triceps skinfold thickness,serum albumin index,serum prealbumin index and Kt/V index of patients were significantly changed.The indexes of patients were continuously improved,and the comparison of data were statistically significant(P < 0.05).The decrease of SGA scores in the observation group before and after treatment was significantly greater than that in the control group,and the difference was significant(P < 0.05). Conclusion Clinical management of uremia patients undergoing peritoneal dialysis through continuous quality improvement can effectively improve the nutritional status and long-term prognosis of patients and help to improve the quality of life of patients,which is worth promoting.
