我院静脉用药调配中心差错分析与防范措施

Error analysis and preventive measures in intravenous drug dispensing center in our hospital

目的通过对我院静脉用药调配中心的差错进行分析,提出防范措施,提高输液质量,保证临床合理用药。方法对我院2017年7月~2018年6月的静脉用药调配中心出现的差错进行汇总、分析,并提出相应的防范措施。结果我院2017年7月~2018年6月共调配输液351999组,发生差错67例,差错率为0.019%。差错发生率呈逐渐下降趋势,差错类别主要分为审方差错、贴签差错、摆药差错、调配差错、退药调配、成品输液复核与运送差错、其他,其中主要差错为调配差错。结论对差错进行汇总、分析,提出相应的防范措施,对防止发生相似的差错尤为重要,有助于提高成品输液的质量,保证患者用药安全,促进临床合理用药。

Objective To put forward the preventive measures to improve the quality of infusion and ensure the clinical rational drug use by analyzing errors of intravenous drug dispensing center in our hospital. Methods The errors of intravenous drug dispensing center in our hospital from July 2017 to June 2018 were summarized and analyzed,and corresponding preventive measures were proposed. Results There were a total of 351999 infusion groups in our hospital from July 2017 to June 2018,with 67 cases of errors.And the error rate was 0.019%.The incidence of errors tended to decrease gradually.Error categories were mainly divided into errors of reviewing the prescription,labeling error,medicine placement error,dispensing error,withdrawal dispensing error,finished infusion reviewing error,delivery error and other errors.The main error was dispensing error. Conclusion It is especially important to summarize and analyze the errors and propose corresponding preventive measures to prevent similar errors from occurring.It can help improve the quality of finished infusion, ensure the safety of patients’ medication and promote clinical rational drug use.
