TP方案联合调强放疗治疗局部晚期鼻咽癌的临床疗效

Clinical Study of TP Regimen Combined with Intensity-modulated Radiotherapy for Patients with Locally Advanced Nasopharyngeal Carcinoma

目的探讨TP方案联合调强放疗治疗局部晚期鼻咽癌的临床疗效。方法选取局部晚期鼻咽癌患者84例,依据治疗方案不同将其分成2组。观察组(42例)采用TP方案治疗,对照组(42例)行PF诱导化疗,2个周期后2组患者均进行调强放疗,结合顺铂80 mg/m2(d1-3)同期化疗,1个周期为21 d,治疗2~3个周期。观察2组患者近期治疗效果和不良反应状况。结果观察组患者鼻咽原发灶与颈部淋巴结转移灶CR人数略高于对照组,但差异无统计学意义(P> 0. 05);观察组患者在放化疗期间发生中性粒细胞、呕吐及脱发几率高于对照组,差异有统计学意义(P <0. 05),其他不良反应无明显差异。结论 TP方案联合调强放疗对局部晚期鼻咽癌患者近期治疗效果比较理想,患者毒副作用能够耐受。

Objective To analyze the clinical efficacy of TP regimen combined with intensity-modulated radiotherapy versus PF regimen combined with intensity-modulated radiotherapy for locally advanced nasopharyngeal carcinoma. Methods 84 patients with locally advanced nasopharyngeal carcinoma,according to different treatment regimens,were divided into the observation group( 42 cases) with TP regimen,and the control group( 42 cases) with PF induction chemotherapy,both groups received IMRT after 2 cycles,combined with cisplatin 80 mg/m2( d1 ~ 3) concurrent chemotherapy,a period of 21 days,treatment period 2 to 3;recent therapeutic effects and adverse conditions of the 2 groups were observed. Results Patients with primary nasopharyngeal and neck lymph node metastasis CR number was slightly higher than that of control group,but the difference was not statistically significant( P > 0. 05);the patients in the observation group during chemotherapy of neutrophils,vomiting and hair loss probability was higher than the control group,the difference was statistically significant( P < 0. 05),there had no significant differences in adverse reactions. Conclusion TP regimen induction chemotherapy combined with concurrent chemoradiotherapy is ideal for short-term treatment of locally advanced NPC patients. The side effects of patients can be tolerated.
