血清中EPCA-2及PSA水平对前列腺癌的诊断价值

Diagnostic Value of Serum EPCA-2 and PSA for Prostate Cancer

目的探讨血清中早期前列腺癌抗原-2(EPCA-2)与前列腺特异性抗原(PSA)对前列腺癌的诊断价值。方法选择前列腺癌患者61例为前列腺癌组,65例良性前列腺增生患者为良性前列腺增生组,同期选择60例非前列腺疾病患者为对照组。采用化学发光法和酶联免疫吸附法检测所有受试者的血清PSA、ERCA-2水平,对比3组受试者的血清EPCA-2、PSA水平;计算各指标单独及联合诊断前列腺癌的阳性率,并对诊断效能进行分析。结果前列腺癌组患者的血清EPCA-2、PSA水平显著高于良性前列腺增生组(P <0. 05),且良性前列腺增生组患者的血清EPCA-2、PSA水平显著高于对照组(P <0. 05)。前列腺癌组患者血清EPCA-2、PSA、EPCA-2+PSA检测阳性率均高于良性前列腺增生组和对照组,且良性前列腺增生组患者的各阳性率均高于对照组(P <0. 05)。EPCA-2、PSA联合诊断前列腺癌的特异性、敏感性均显著高于EPCA-2、PSA单一检测。结论与单一指标诊断效果相比,血清EPCA-2与PSA联合诊断前列腺癌的敏感性、特异性均较高,可推荐使用于前列腺癌的早期诊断过程中。

Objective To analyze the diagnostic value of serum prostate cancer antigen-2( EPCA-2) and prostate specific antigen( PSA) for prostate cancer. Methods 61 patients with prostate cancer were enrolled in the prostate cancer group,65 patients with benign prostatic hyperplasia were in the benign prostatic hyperplasia group,and 60 patients with non-prostate disease were selected as the control group. Serum PSA and ERCA-2 levels were measured in all subjects by chemiluminescence and enzyme-linked immunosorbent assay. Serum EPCA-2 and PSA levels were compared among the 3 groups. The positive rate of each indicator for prostate cancer alone and in combination was calculated,and the diagnostic efficacy was analyzed. Results Serum EPCA-2 and PSA levels were significantly higher in the prostate cancer group than in the benign prostatic hyperplasia group( P <0. 05),and serum EPCA-2 and PSA levels were significantly higher in the benign prostatic hyperplasia group than of the control group( P < 0. 05). The positive rates of serum EPCA-2,PSA and EPCA-2 + PSA in prostate cancer patients were higher than those of benign prostatic hyperplasia group and the control group,and the positive rates of patients with benign prostatic hyperplasia group were higher than those of the control group( P < 0. 05). The specificity and sensitivity of EPCA-2 and PSA combined with diagnosis of prostate cancer were significantly higher than those of EPCA-2 and PSA. Conclusion Compared with the diagnosis of single index,serum EPCA-2 and PSA combined with high sensitivity and specificity for the diagnosis of prostate cancer are recommended. It can be recommended for early diagnosis of prostate cancer.
