TTI技术在急性广泛前壁心肌梗死患者左心收缩功能评价中的应用价值

摘要 目的探讨组织追踪成像技术(TTI)在急性广泛前壁心肌梗死患者左心收缩功能评价中的应用价值。方法选择2016年6月—2017年6月吉林省吉林市中心医院收治的30例急性广泛前壁心肌梗死患者为观察组,选择同期健康体检者30例为对照组,均采用双平面Simpson’s法测量左室射血分数(LVEF),并应用TTI获得心肌运动速度-位移曲线,计算左室壁各节段的最大位移(Ds)和平均最大位移(mDs),并分析各节段mVs、mDs与LVEF的相关性。结果观察组患者LVEF水平明显低于对照组,差异有统计学意义(P<0.01)。观察组患者左室壁各节段mDs均明显低于健康体检的对照组,差异均有统计学意义(P<0.01)。所有受检者左室心肌基底段水平及中部乳头肌水平mVs、mDs与LVEF存在相关性(P<0.05或P<0.01)。结论 TTI技术测量的左室壁心肌基底段水平及中部乳头肌水平mVs、mDs与LVEF存在明显的相关性,能快速、准确地对心肌梗死患者的左心收缩功能进行定量评价,值得临床推广。 OBJECTIVE To investigate the application value of left ventricular systolic function evaluation in acute extensive anterior wall myocardial infarction patients with Tissue Tracking Imaging(TTI)technology. METHODS To select 30 cases acute extensive anterior wall myocardial infarction patients of target hospital in 2016.6-2017.6 as the observation group,and select 30 cases healthy controls as the control group,they were all adopted double plane Simpson’s method to measure the left ventricular ejection fraction(LVEF),adopted TTI technology to acquire the moving velocity-displacement curve,calculated the maximum displacement(Ds)and average maximum displacement(mDs)of the segmental left ventricular wall,and analyzed the correlation between the mVs,mDs and LVEF in each segment. RESULTS The LVEF level of the observation group patients was significantly lower than the control group,the difference had statistical significance(P<0.01).The average maximum displacement(mDs)of the segmental left ventricular wall of the observation group were significantly lower than the control group,the difference had statistical significance(P<0.01).There was an correlation between the mVs,mDs and LVEF of left ventricular myocardium basal segment and central papillary muscle of all the examinee(P<0.05 or P<0.01).CONCLUSION There was an correlation between the mVs,mDs and LVEF of left ventricular myocardium basal segment and central papillary muscle measuring with TTI technology,it could quantitatively evaluate the left ventricular systolic function of the myocardial infarction patients quickly and accurately,which was worthy of clinical promotion.
