摘要
目的探讨微小RNA(microRNA)-199a-5p在老年原发性高血压患者中的表达及临床价值。方法选取老年原发性高血压患者120例为观察组,其中高血压1级26例,2级61例,3级33例,同期健康老年人50例为对照组。收集入选患者一般临床资料,检测颈动脉内膜中层厚度、LVEF、左心室舒张末期容积(LVEDV)及左心室收缩末期容积(LVESV),计算肾小球滤过率(GFR),检测microRNA-199a-5p表达。采用Pearson相关性分析。结果与对照组比较,观察组颈动脉内膜中层厚度、LVEDV和LVESV明显升高,LVEF和GFR明显降低(P<0.05,P<0.01)。观察组microRNA-199a-5p相对表达水平明显高于对照组(3.09±0.62 vs 0.98±0.23,P<0.05),且高血压1级、2级、3级患者microRNA-199a-5p相对表达水平呈明显升高趋势(2.22±0.54 vs 2.81±0.55 vs 4.31±0.56,P<0.05),明显高于对照组(P<0.05)。高血压患者microRNA-199a-5p相对表达水平与颈动脉内膜中层厚度、LVEDV、LVESV和收缩压呈正相关,与LVEF呈负相关(P<0.05,P<0.01),与GFR和舒张压无关(P>0.05)。结论老年原发性高血压患者microRNA-199a-5p增高,与心肌损伤、动脉粥样硬化有关。
Objective To study the expression of microRNA-199 a-5 p in elderly essential hypertension(EH)patients and its clinical value.Methods One hundred and twenty elderly EH patients admitted to our hospital served as an observation group and 50 healthy elderly people served as a control group.The general clinical data were recorded,the carotid IMT,LVEDV,LVESV were measured,the LVEF and GFR were calculated,the microRNA-199 a-5 p expression was detected in two groups.Results The expression level of microRNA-199 a-5 p was significantly higher,the carotid IMT was significantly thicker,the LVEDV and LVESV were significantly larger while the LVEF and GFR were significantly lower in observation group than in control group(P<0.05,P<0.01).The expression level of microRNA-199 a-5 p showed a significant trend to increase in grade 1-3 hypertension patients(2.22±0.54 vs 2.81±0.55 vs 4.31±0.56,P<0.05)and was significantly higher in observation group than in control group(P<0.05).The expression of microRNA-199 a-5 p was positively related with the carotid IMT,LVEDV,LVESV and SBP,negatively related with the LVEF(P<0.05,P<0.01),and not related with the GFR and DBP in elderly EH patients(P>0.05).Conclusion Elevated microRNA-199 a-5 p expression is related with myocardial injury and atherosclerosis in elderly EH patients.
