摘要
目的:探讨锥形束CT(conebeam,CBCT)在各个轴面测量根管工作长度的准确性。方法:将150颗单根管离体恒牙分为5组,分别是实际长度测量组、根尖定位仪测量组,CBCT近远中面测量组、颊舌面测量组、最弯曲面测量组,分别用实际测量法,根尖定位仪法及CBCT软件测量法测量150颗离体牙的根管工作长度,验证CBCT在各个轴面测量根管工作长度的准确性。结果:5个组测量长度差异有统计学意义(P<0.05),两两比较结果显示最弯曲面组与实际长度测量组差异无统计学差异(P>0.05);根管弯曲数目为0时,差异无统计学意义(P>0.05),当根管弯曲数目为1、2个弯曲时,测量结果具有统计学差异(P<0.05),两两比较结果显示最弯曲面组与实际长度测量组测量结果无统计学差异(P>0.05);弯曲角度为5°~20°时,测量结果具有统计学差异(P<0.05),两两比较结果显示最弯曲面组与实际长度测量组测量结果无统计学差异(P>0.05)。结论:CBCT测量根管工作长度的准确性高,且测量结果不受根管弯曲数目、角度的影响,可能提高根管治疗的成功率。
Objective: To investigate the accuracy of measuring the working length of root canals by cone-beam computed tomography(CBCT) on each axial plane. Methods: A total of 150 extracted single-rooted permanent teeth were divided into the following five groups: actual length measurement group, root apex locator measurement group,and CBCT measurement groups of the mesiodistal plane, the buccolingual plane, and the most curved plane. The above groups were evaluated for the working length of root canals for the 150 extracted teeth were measured using the actual measurement method, root apex locator measurement method, and CBCT software measurement method to verify the accuracy of measuring the working length of root canals by CBCT on each axial plane. Results: There was a significant difference in measured length between the five groups(P < 0.05);however, pairwise comparison showed that no significant difference in measured length was observed between the most curved plane group and the actual length measurement group(P > 0.05). When the number of root canal curves was 0, the difference was not statistically significant(P > 0.05);when the number of root canal curves was 1 or 2, a significant difference in measurement result was observed(P < 0.05);pairwise comparison showed that no significant difference in measurement result was observed between the most curved plane group and the actual length measurement group(P > 0.05). When the bending angle was 5~20°, there was a significant difference in measurement result(P < 0.05);pairwise comparison showed that there was no significant difference in measurement result between the most curved plane group and the actual length measurement group(P > 0.05). Conclusion: CBCT has high accuracy in measuring the working length of root canals, and the measurement result is independent of the curve number and angle of root canals, so it may improve the success rate of root canal therapy.
