华法林对心房颤动患者凝血功能及血流变指标影响浅析

Effect of warfarin on the coagulation function and hemorheological parameters in patients with atrial fibrillation: A preliminary analysis

目的:探讨华法林对房颤患者凝血功能及血液流变学参数的影响。方法:将2017年10月至2018年10月收治的106例老年房颤患者分为观察组(53例)和对照组(53例)。对照组采用常规治疗,观察组在对照组基础上采用华法林治疗。两组均接受4周的治疗。比较两组治疗前后的疗效、心电图变化、凝血功能及血液流变学。结果:观察组总有效率(92.45%)高于对照组(73.58%),差异有统计学意义(P<0.05);两组治疗后凝血酶时间(TT)和激活凝血酶时间(APTT)均较治疗前升高,差异有统计学意义(P<0.05);治疗后观察组TT和APTT均高于对照组(73.58%)。两组治疗后血浆黏度、红细胞聚集指数和纤维蛋白原均较治疗前下降(P<0.05)。治疗后观察组血浆黏度、红细胞聚集指数、纤维蛋白原均低于对照组(P<0.05)。结论:华法林治疗房颤有效,能改善房颤患者的凝血和血液流变学功能。

Objective: To investigate the effect of warfarin on coagulation function and hemorheological parameters in patients with atrial fibrillation. Methods: A total of 106 elderly patients with atrial fibrillation who were admitted to our hospital from October 2017 to October 2018 were enrolled and randomly divided into observation group and control group, with 53 patients in each group. The patients in the control group were given routine treatment, and those in the observation group were treated with warfarin in addition to the treatment in the control group. The course of treatment was 4 weeks for both groups. The two groups were compared in terms of clinical outcome, electrocardiographic findings, coagulation function, and hemorheology before and after treatment. Results: The observation group had a significantly higher overall response rate than the control group (92.45% vs 73.58%, P < 0.05). Both groups had significant increases in thrombin time (TT) and activated partial thromboplastin time (APTT) after treatment (P < 0.05), and the observation group had significantly longer TT and APTT than the control group (P < 0.05). Both groups had significant reductions in plasma viscosity, erythrocyte aggregation index, and fibrinogen after treatment (P < 0.05), and the observation group had significantly lower plasma viscosity, erythrocyte aggregation index, and fibrinogen than the control group (P < 0.05). Conclusion: Warfarin has a good therapeutic effect in patients with atrial fibrillation and can improve their blood coagulation and hemorheological function.
