锁定加压钢板螺钉与交锁髓内钉内固定治疗股骨干骨折的效果观察

Therapeutic effect of locking compression plate screw and interlocking intramedullary nail in the treatment of femoral shaft fracture

目的探讨锁定加压钢板螺钉与交锁髓内钉内固定治疗股骨干骨折的临床效果。方法将94例成人股骨干骨折患者随机分为两组,各47例。A组行交锁髓内钉内固定治疗,B组行锁定加压钢板螺钉治疗。比较两组患者围术期指标、膝关节功能及并发症发生情况。结果A组术中出血量低于B组(P<0.01),骨折愈合时间、骨痂出现时间及住院时间均短于B组(P<0.01);A组膝关节功能恢复优良率高于B组(P<0.05);A组并发症总发生率低于B组(P<0.01)。结论交锁髓内钉内固定治疗股骨干骨折效果显著,具有出血量少、住院时间短、骨折愈合快、并发症少等优势,术后膝关节功能恢复好。

Objective To investigate the efficacy of locking compression plate screws and interlocking intramedullary nailing in treating adult femoral shaft fractures. Methods A total of 94 patients with femoral shaft fractures were randomly divided into two groups,47 cases in each group.Group A received interlocking intramedullary nailing,and group B did locking compression plate screws.Perioperative indicators,knee function and postoperative complications were compared between the two groups. Results Group A achieved a lower intraoperative bleeding,and shorter length of hospital stay,the time of osteophyte of fracture healing compared with group B( P <0.01).The distinguished achiever ratio of knee function recovery in group A was statistically higher than that of group B( P <0.05).There existed a statistical difference in the incidence of complications between the two groups( P <0.01). Conclusion Interlocking intramedullary nailing could reduce intraoperative bleeding,hospitalization and complications and promote fracture healing,following a better knee joint function in the treatment of adult femoral shaft fractures.
