冠心病患者血清急性时相反应蛋白及NF-κB水平变化的意义 被引量:5

Association of serum concentrations of acute phase reaction proteins and NF-κB with the development of coronary heart disease

摘要 目的探讨冠心病血清急性时相反应蛋白[C反应蛋白(CRP)、血清淀粉样蛋白-A(SAA)、前白蛋白(PA)]及单核细胞核因子-κB(NF-κB)的变化与冠心病病情的关系。方法选取冠心病患者120例(冠心病组),均接受冠脉造影检查;同时,选取健康志愿者100例(对照组),检测比较两组的血清CRP、SAA、PA和NF-κB浓度。结果冠心病组CRP、SAA和NF-κB高于对照组(P<0.01),而PA低于对照组(P<0.01);急性心肌梗死(AMI)患者CRP、SAA和NF-κB高于不稳定型心绞痛(UAP)和稳定性心绞痛(SAP)患者(P<0.01),而PA低于UAP和SAP(P<0.01);重度狭窄者CRP、SAA和NF-κB高于轻度和中度狭窄者(P<0.01),而PA低于轻度和中度狭窄者(P<0.01);CRP、SAA和NF-κB与Gensini评分呈正相关(P<0.05),PA与Gensini评分呈负相关(P<0.05)。结论冠心病患者CRP、SAA及NF-κB水平明显升高,而PA明显降低,与冠心病的类型及冠脉狭窄程度有关。 Objective To investigate the correlation of the concentrations of the acute phase proteins(APPs)such as C-reactive protein(CRP),serum amyloid-A(SAA)and prealbumin(PA),monocyte nuclear factor B(NF-κB)and coronary heart disease. Methods A total of 120 patients diagnosed with coronary heart disease by coronary angiogram were randomly selected as coronary heart disease group and 100 healthy volunteers done as control group meanwhile.The concentrations of CRP,SAA,PA and NF-κB were compared between the two groups. Results Coronary heart disease group achieved significantly higher concentrations of CRP,SAA and NF-κB than those of control group ( P <0.01),but a lower level of PA than that of control group( P <0.01).The CRP,SAA and NF-κB concentrations of AMI patients were significantly higher than those of UAP and SAP patients( P <0.01),while PA was significantly lower than that of UAP and SAP cases( P <0.01).Significantly higher CRP,SAA and NF-κB levels and a lower PA of severe stenosis patients occurred compared with those of mild and moderate stenosis patients( P <0.01).Serum CRP,SAA and NF-κB were positively correlated with Gensini score( P <0.05).However,PA was negatively correlated with Gensini score( P <0.05). Conclusion Serum concentrations of CRP,SAA and NF-κB increased and PA decreased significantly in the patients with coronary heart disease,which could reflex a type of coronary heart disease and stenosis severity of coronary artery.
