上海市某社区卫生服务中心骨质疏松药物使用现状分析

Analysis of the current situation of osteoporosis drug use in a community health service center in Shanghai

目的:从药物消费角度探讨社区骨质疏松疾病的防治现状。方法:从陆家嘴社区卫生服务中心的计算机管理系统调取2016-2018年所有骨质疏松药物的使用记录,收集药物的名称、规格、零售价、消耗数量及用药金额等信息,根据信息计算药物的用药频度及日均费用。结果:陆家嘴社区卫生服务中心主要使用4大类共8种骨质疏松药物,2016-2018年的骨质疏松药物消耗总金额分别为116.52万元、128.48万元及92.34万元,其中钙制剂类的消耗金额稳居首位;阿仑膦酸钠片的消耗金额由2016、2017年的第5位上升到2018年的第4位;仙灵骨葆胶囊2018年的消耗金额较2016、2017年有显著下降,消耗金额的单个用药排序也由第1位下降到第3位。结论:陆家嘴社区卫生服务中心的骨质疏松药物品种偏少,缺乏降钙素类、选择性雌激素受体调节剂和甲状旁腺激素片段类等骨质疏松药物。社区卫生服务中心应进一步加强医护人员在管理、筛查及防治骨质疏松症方面的关口作用,从而全面提升骨质疏松患者的生命质量。

Objective: To discuss the current situation of prevention and treatment of osteoporosis in the community from the perspective of drug consumption. Methods: The records of all osteoporotic drugs used from 2016 to 2018 were retrieved from the computer management system in Lujiazui Community Health Service Center to collect information such as name, specification, retail price, quantity consumed and drug amount, etc. and the defined daily dose system(DDDs) and the defined daily dose consumption(DDDc) of osteoporosis drugs used also were calculated. Results: Lujiazui Community Health Service Center mainly used 8 kinds of osteoporosis drugs in 4 categories. From 2016 to 2018, the total consumption of osteoporotic drugs was 116.52 million Yuan, 128.48 million Yuan and 923.4 million Yuan, respectively, among which the consumption of calcium preparations ranked first;the consumption of alendronate sodium tablets increased from the 5th place in 2016 and 2017 to the 4th place in 2018;the consumption of Xianling gubao capsule in 2018 was significantly lower than that in 2016 and 2017, and the single medication order of consumption amount also dropped from the first to the third. Conclusion: Lujiazui Community Health Service Center has a small number of osteoporosis drugs, and lacks osteoporosis drugs such as calcitonin, selective estrogen receptor modulators and parathyroid hormone fragments. The community health service center should further strengthen the critical role of medical staff in the management, screening and prevention of osteoporosis, so as to comprehensively improve the quality of life of patients with osteoporosis.
