糖尿病肾病患者血清LncRNA EXOC7的表达及意义

Expression and Significance of Serum LncRNA EXOC7 in Patients with Diabetic Nephropathy

目的探讨LncRNA EXOC7(EXOC7)在糖尿病肾病患者血清中的表达及其临床意义。方法通过qRT-PCR法检测120例Ⅱ型糖尿肾病患者、120例无肾脏损害的Ⅱ型糖尿病患者及120例健康对照组血清中EXOC7的表达,分析其与患者蛋白尿、肾小球滤过率等临床病理参数的相关性。结果与健康对照组比较,EXOC7在Ⅱ型糖尿病患者及糖尿病肾病患者中的表达明显升高,差异有统计学意义(P<0.05);与Ⅱ型糖尿病患者比较,糖尿病肾病患者血清中EXOC7的表达明显增高,差异具有统计学意义(P<0.05)。EXOC7在合并大量蛋白尿的糖尿病肾病患者中的表达明显高于合并微量蛋白尿患者,差异有统计学意义(P<0.05)。血清LncRNA EXOC7的表达与蛋白尿、空腹血糖、糖化血红蛋白、甘油三脂及低密度脂蛋白(LDL)的表达量呈正相关关系,差异有有统计学意义(P均<0.05);而与肾小球滤过率呈负相关关系(P<0.001)。多因素分析发现,空腹血糖、eGFR、HbA1c、总胆固醇、甘油三酯、低密度脂蛋白是影响患者蛋白尿的因素(P<0.05)。结论 EXOC7的高表达与Ⅱ型糖尿病患者糖尿病肾病的发展有关,有望成为预测糖尿病肾病患者预后的生物标志物。

Objective To investigate the expression of LncRNA EXOC7(EXOC7)in serum of patients with diabetic nephropathy and its clinical value.Methods The expression of EXOC7 in serum of 120 patients with type 2 diabetic nephropathy,120 patients with type 2 diabetes without renal damage and 120 healthy controls was analyzed by qRT-PCR.The correlation between EXOC7 expression and clinicopathological parameters such as albuminuria and glomerular filtration rate was analyzed.Results Compared with healthy controls,the expression of EXOC7 was significantly increased in patients with type 2 diabetes and diabetic nephropathy(P<0.05).The expression of EXOC7 in serum of patients with diabetic nephropathy was significantly higher than that of patients with type 2 diabetes mellitus(P<0.05).The expression of EXOC7 in diabetic nephropathy patients with massive proteinuria was significantly higher than that in patients with microalbuminuria(P<0.05).The expression of serum LncRNA EXOC7 was positively correlated with the expression of albuminuria,fasting blood glucose,glycated hemoglobin(HbA1c),triglyceride and low density lipoprotein(LDL)(P<0.001),but negatively correlated with renal filtration rate(eGFR)(P<0.001).Multivariate analysis found that fasting blood glucose,eGFR,HbA1c,total cholesterol,triglyceride,and low-density lipoprotein were factors affecting proteinuria in patients(P<0.05).Conclusion The high expression of EXOC7 is associated with the development of diabetic nephropathy in patients with type 2 diabetes and is expected to be a biomarker for predicting the prognosis of patients with diabetic nephropathy.
