雄激素和脂代谢水平变化与多囊卵巢综合征患病关系研究

摘要 目的探讨多囊卵巢综合征(PCOS)患者雄激素和脂代谢的水平及意义。方法选取2017年1月至2018年6月在我院治疗的PCOS患者106例(PCOS组),同时选取健康体检妇女110例作为对照组,检测两组血脂及雄激素水平。结果 PCOS组体质指数、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、总睾酮(T)和雄烯二酮(A2)分别为(24.56±4.03) kg/m^2、(1.64±0.45) mmol/L、(4.60±0.72) mmol/L、(3.20±0.87) nmol/L和(3.41±0.98) ng/mL,明显高于对照组,差异有统计学意义(P<0.05);而高密度脂蛋白胆固醇(HDL-C)为(1.00±0.30) mmol/L,明显低于对照组,差异有统计学意义(P<0.05)。PCOS组≥25kg/m^2患者TG、TC、LDL-C、T和A2分别为(1.84±0.35) mmol/L、(5.10±0.81) mmol/L、(2.56±0.64) mmol/L、(3.44±0.97) nmol/L和(3.52±0.81) ng/m L,明显高于<25kg/m^2患者,差异有统计学意义(P<0.05);而HDL-C为(0.92±0.31) mmol/L,明显低于<25kg/m^2患者,差异有统计学意义(P<0.05)。T与体质指数、TC呈正相关(r=0.341和0.355,P<0.05),A2与体质指数呈正相关(r=0.325,P<0.05)。结论 PCOS患者雄激素和脂代谢水平明显异常,其中雄激素T、A2与患者体质指数及血脂有一定关系。 Objective To investigate the changes and meanings of androgen and lipid metabolism in patients with polycystic ovary syndrome(PCOS).Methods From January 2017 to June 2018,106 PCOS patients(PCOS group)in our hospital were enrolled,at the same time,110 healthy women were selected as the control group.The blood lipid and androgen levels were measured.Results In PCOS group,the body mass index(BMI),triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),total testosterone(T)and androstenedione(A2)respectively were (24.56±4.03)kg/m^2 、(1.64±0.45)mmol/L、(4.60±0.72)mmol/L、(3.20±0.87)nmol/L and (3.41±0.98)ng/mL,which were significantly higher than control group(P<0.05).However,high-density lipoprotein cholesterol(HDL-C)was(1.00±0.30)mmol/L,which was significantly lower than control group(P<0.05).When BMI ≥ 25kg/m^2 in PCOS group,TG,TC,LDL-C,T and A2 respectively were(1.84±0.35)mmol/L,(5.10±0.81)mmol/L,(2.56±0.64)mmol/L,(3.44±0.97)nmol/L and(3.52±0.81)ng/ml,which is significantly higher than the patients who BMI<25 kg/m^2 (P<0.05).Moreover,HDL-C was(0.92±0.31)mmol/L,which was significantly lower than the patients who BMI<25 kg/m^2 (P<0.05).T was positively correlated with BMI and TC(r=0.341和0.355, P<0.05),and A2 was positively correlated with BMI (r=0.325, P<0.05).Conclusion The androgen and lipid metabolism are obviously abnormal in PCOS patients.And androgen T and A2 are related to the body mass index and blood lipids.
