不同剂量右美托咪定对颌骨囊肿手术患者全麻苏醒质量的影响

Effect of Different Doses of Dexmedetomidine on Recovery Quality of General Anesthesia in Patients Undergoing Jaw Cyst Surgery

目的:探讨不同剂量右美托咪定对颌骨囊肿手术患者全麻苏醒质量的影响。方法:选取2017年8月至2019年5月厦门医学院附属口腔医院收治的颌骨囊肿手术患者90例作为研究对象,随机分为A、B、C 3组,每组30例。分别给予A组、B组0.4μg/kg、0.8μg/kg的右美托咪定,给予C组等量生理盐水。比较3组患者在T0(麻醉前)、T1(拔管即刻)、T2(拔管后10 min)时的心率、收缩压、舒张压;苏醒期间躁动、呛咳程度及发生率情况。结果:在T0时刻,3组的心率及血压差异无统计学意义(P>0.05)。与T0时刻比较,3组T1、T2时刻的心率、血压均有明显升高(P<0.05)。在T1、T2时刻,A、B2组心率、血压明显优于C组,且B组优于A组,差异有统计学意义(P<0.05)。与C组比较,A组躁动发生情况基本相似(P>0.05),呛咳程度及发生率降低,但差异无统计学意义(P>0.05);与C组比较,B组躁动及呛咳程度及发生率,也明显较低(P<0.05)。结论:在颌骨囊肿手术结束之前,向患者泵入0.8μg/kg的右美托咪定,可明显改善患者的苏醒质量,稳定其在全麻期间的血流动力学指标。

Objective: To investigate the effect of different doses of dexmedetomidine on the quality of general anesthesia recovery in patients undergoing jaw cyst surgery. Methods: Ninety patients with jaw cyst treated in our hospital were randomly divided into group A,B and C with 30 cases in each group.Group A and B were given dexmedetomidine of 0.4 and 0.8 μg/kg respectively,and group C was given the same amount of saline.Heart rate,systolic blood pressure and diastolic blood pressure at T0(before anesthesia),T1(immediately after extubation)and T2(10 minutes after extubation)were compared among the three groups. Results: There was no significant difference in heart rate and blood pressure among the three groups( P >0.05).Compared with the time of T0,the heart rate and blood pressure at T1 and T2 in the three groups increased significantly( P <0.05).At T1 and T2,heart rate and blood pressure in group A and B were significantly better than those in group C,and group B was better than group A,with statistical significance( P <0.05).Compared with group C,the occurrence of restlessness in group A was similar( P >0.05),the degree and incidence of choking cough decreased,but the difference was not significant( P >0.05);compared with group C,the degree and incidence of restlessness and choking cough in group B were also significantly lower( P <0.05). Conclusion: Pumping dexmedetomidine 0.8 μg/kg into patients before the operation of jaw cyst can improve the quality of recovery and stabilize the hemodynamic parameters during general anesthesia.
