平肝息痛汤联合止痉散外敷治疗肝阳上亢型偏头痛临床分析

Clinical observation of decoction for calming liver and stopping pain combined with powder for stopping spasm in the treatment of migraine with Hyperactivity of Liver Yang

〔目的〕分析自拟平肝息痛汤联合止痉散外敷治疗肝阳上亢型偏头痛的临床疗效。〔方法〕70例患者随机分为治疗组和对照组,每组35例。对照组口服盐酸氟桂利嗪胶囊10mg,每晚睡前服1次;治疗组用平肝息痛汤加减,每日1剂水煎,早晚各1次温服,同时止痉散外敷太阳穴及前额病痛处,每日换药1次。两组均连用4周,观察头痛改善情况,比较临床疗效。〔结果〕①治疗后患者的VAS评分较治疗前改善,头痛发作次数和头痛持续时间较前减少,治疗组优于对照组(P<0.05);②治疗组总有效率为94.3%,共治愈20例,有效13例,无效2例;对照组总有效率为82.9%,共治愈14例,有效15例,无效6例,差异有统计学意义(P<0.05)。〔结论〕本法治疗肝阳上亢型偏头痛疗效显著,简便易行,且无毒副作用,值得临床推广应用。

〔Objective〕To observe the clinical effect of self-made Decoction for calming liver and stopping pain combined with Powder for stopping spasm on migraine with hyperactivity of liver-yang.〔Methods〕70 patients were randomly divided into treatment group and control group, 35 cases in each group. The control group took flunarizine hydrochloride capsule 10 mg orally, once every night before going to bed;The treatment group was treated with Decoction for calming liver and stopping pain, decocted in water once a day, taken warmly in the morning and evening, and Powder for stopping spasm was applied to the temple and forehead pain area, and changed dressings once a day. Both groups were used continuously for 4 weeks to observe the improvement of headache and compare the clinical efficacy.〔Results〕①After treatment, VAS scores of patients were improved, the number of headache attacks and the duration of headache were decreased. The treatment group was better than the control group (P < 0. 05).②The total effective rate of the treatment group was 94. 3%. There were 20 cases cured, 13 cases effective and 2 cases ineffective. The total effective rate of the control group was 82. 9%. There were 14 cases cured, 15 cases effective and 6 cases ineffective. The difference was statistically significant (P < 0. 05).〔Conclusion〕This method has obvious curative effect on migraine with hyperactivity of liver-yang. It is simple and easy to operate, and has no toxic side effects. It is worth popularizing and applying in clinic.
