生物反馈治疗35例肛提肌综合征的临床疗效分析

Clinical analysis of 35 cases of levator ani syndrome treated with biofeedback

目的评估生物反馈治疗肛提肌综合征的临床疗效。方法对2015年6月至2018年1月北京市肛肠医院收治的35例肛提肌综合征患者给予生物反馈治疗,每疗程5次,连续两个疗程,评估其疼痛缓解程度及疼痛评分情况。结果本组35例患者中,21例(60%)患者疼痛完全缓解,6例(17%)患者暂时缓解,8例(23%)患者无效;治疗前疼痛评分为(5.83±1.01)分,治疗后2周疼痛评分为(3.06±1.96)分,治疗后1个月疼痛评分为(3.31±2.08)分,治疗后3个月疼痛评分为(3.14±2.42)分,不同时点的疼痛评分比较差异有统计学意义(F=24.288,P<0.001),即治疗后2周、治疗后1个月、治疗后3个月疼痛评分较治疗前均降低差异有统计学意义(均P<0.001);不同时点疼痛评分程度等级比例结果显示,生物反馈治疗后2周,轻度疼痛患者比例较治疗前提升,短期疼痛缓解效果较好,随着时间延长(治疗后1个月、治疗后3个月),轻度疼痛患者比例下降。所有患者未发生治疗相关不良反应。结论生物反馈治疗能够有效缓解肛提肌综合征患者的临床症状,减轻疼痛。

Objectives To investigate the effect of biofeedback for levator ani syndrome. Methods Thirty-five patients with le vator ani syndrome received biofeedback treatment between June 2016 and January 2018 at Beijing Rectum Hospital. Patients re ceived two cycles of treatment, which involved ten biofeedback treatments. Relief of pain and pain scores were evaluated. Re- sults Of the 35 patients, 21 patients (60%) had complete relief of pain, 6 (17%) temporary relief, and 8 (23%) no relief. The pre-treatment pain score was (5.83±1.01). The pain score after twoweeks of treatment was (3.06±1.96),(3.31±2.08) after one month of treatment, and (3.14±2.42) after three months of treatment. There was significant decrease in pain scores at all post- treatment time points compared with pre-treatment (F = 24.288,P < 0.001). Two weeks after biofeedback treatment, proportions of patients with mild pain increased compared with pre-treatment, indicating significant short-term pain relief. Proportions of pa tients with mild pain decreased after one ,and three months of treatment. No adverse event was reported. Conclusion Biofeed back treatment can effectively relieve symptoms and reduce pain in patients with levator ani syndrome.
