尿激酶联合序贯抗凝疗法治疗老年肺栓塞及对生存质量的影响

To explore the effect of urokinase combined with sequential anticoagulation therapy on the quality of life in elderly patients with pulmonary embolism

目的探究尿激酶注射液联合序贯抗凝法治疗老年肺栓塞的疗效及其在提高生活质量方面的价值。方法对照分析我院72例老年肺栓塞患者在不同治疗方法下的临床效果和生存质量。结果在治疗效果上,观察组的有效率为97.22%,对照组的有效率为77.77%,观察组显著高于对照组(P<0.05),有显著性差异。在临床指标方面,治疗前两组患者在心率、二氧化碳分压、动脉血氧分压等指标上无显著性差异(P>0.05),但治疗后两组指标均有不同程度的改善,观察组患者指标改善程度明显优于对照组(P<0.05),差异具有统计学意义。在不良反应方面,观察组患者的发生率为27.77%,对照组发生率为19.44%,观察组略高于对照组(P>0.05),但差异不具统计学意义。在生活质量方面,两组在躯体疼痛、情感职能等方面无显著性差异(P>0.05),在身体健康、社会职能、精力等方面,观察组患者得分显著高于对照组,有显著性差异(P<0.05)。结论尿激酶注射液联合序贯抗凝法治疗老年肺栓塞具有良好的效果,用药安全,能有效提高患者生存质量,建议在临床推广应用。

Objective To explore the efficacy of urokinase injection combined with sequential anticoagulation in the treatment of elderly patients with pulmonary embolism and its value in improving the quality of life. Methods The clinical effects and quality of life of 72 elderly patients with pulmonary embolism in our hospital under different treatment methods were compared and analyzed. Results In the treatment effect,the effective rate of the observation group was 97.22%,and that of the control group was 77.77%.The observation group was significantly higher than that of the control group ( P <0.05),and the difference was statistically significant.In terms of clinical indicators,there was no significant difference in heart rate,carbon dioxide partial pressure,arterial oxygen partial pressure and other indicators between the two groups before treatment ( P >0.05),but after treatment,the indicators of the two groups were improved in varying degrees.The improvement of the indicators in the observation group was significantly better than that in the control group ( P <0.05),and the difference was statistically significant.In terms of adverse reactions,the incidence of patients in the observation group was 27.77%,and that in the control group was 19.44%.The incidence in the observation group was slightly higher than that in the control group ( P >0.05),but the difference was not statistically significant.In terms of quality of life,there was no significant difference in physical pain and emotional function between the two groups ( P >0.05).In terms of physical health,social function and energy,the scores of patients in the observation group were significantly higher than those in the control group ( P< 0.05). Conclusion Urokinase injection combined with sequential anticoagulation therapy has good effect in the treatment of elderly patients with pulmonary embolism.It is safe to use drugs and can effectively improve the quality of life of patients.It is suggested that it be popularized in clinical practice.
